January 7, 2003 Peace Health Property Planning & Development Attn: Tamara Miller Re: Pre-demolition asbestos survey 3479 Baldy View Road, Springfield, Oregon On January 4, 2003 an inspection and site survey for asbestos containing building materials was conducted at the above-mentioned address. From the homogeneous list of building materials established, Roofing, ceiling and wall composites and several types of flooring materials were sampled and analyzed by an independent laboratory, Magnum Environmental Services. The results indicated that no asbestos was present in any of the materials sampled, The house, following this exercise, is considered ready for demolition pursuant to the rules specified in DEQ Division 340-248-0270 and LRAPA's Title 43-015-10- A&B regarding asbestos.
