requested and approved, and all debris is removed from the site.
2 Sanitary Sewer Cap: Capped within five (5) feet of the property line and capped with an approved material as
required by the code.

information hereon is true and correct, and I further certify that any and all work performed shall be done in accordance with
the Ordinances of the City of Springfield and the Laws of the State of Oregon pertaining to the work described herein, and
that NO OCCUPANCY will be made of any structure without permission of the Community Services Division, Building Safety.

The applicant is hereby notified that any redevelopment of the subject site must comply,
with all of the applicable laws, codes, ordinances, policies and plans in effect at the time ,
the redevelopment proposal is accepted as complete for City review. 1bis would inClude
, correction of substandard conditions associated with the'present development. Examples
. of such corrections may include modification of inadequate drainage facilities;
compliance with building set-backs from property lines; correction of substandard
sidewalks and street improvements, including driveway width and placement; and other

DEMOLITION PERMIT APPLICATIONS
Your demolition permit is currently being .processed. There may be a slight
delay, of up to 2 working days for small structures, due to the time required
to review the history of the structure to determine if it needs to be documented
before demolition. This documentation is for archival purposes only and will
not affect the granting of the demolition permit. If the structure is very large.
or complicated the documentation process may take up to a maximum of 4 working
days. Documentation will consist of photographing thebuilding, taking
measurements and making scaled drawings. The documentation will be undertaken
by the City at no cost to you. Documentat ion is be i ng done on a 11 structures
dated prior to 1940 that may have historic importance to the City's development.'
.~ T~IS DOCUMENTATION WI~l NOT IMPEDE THE DEMOLITION PROCESS.
An age cut-off of 1940 was chosen because this is the date that the National
Parks Service and the Springfield Development Code use to determine potential
historical significance.

Re: Pre-demolition asbestos survey House, attached carport and storage
buildings at: 3492 Baldy View Lane, Springfield, Oregon
On February 14, 2004 an inspection and site survey for asbestos containing
building materials was conducted at the above-mentioned address. On April 2,
2004 a confirmatory exercise was conducted to confirm the reports accuracy and
adequacy. From the homogeneous list of building materials established in this
exercise, 22 types of materials were sampled and analyzed by an independent
laboratory, E.H.S. LLC. The results indicated that asbestos was present in 3 of
the flooring materials and Thermal Systems Insulation found in the crawl space
under the kitchen and bathroom.
The asbestos found in the flooring materials in the kitchen, dining room and
hallway amounted to approx. 315 sq. ft The TSI was approx. 18 sq. ft. in total.
These materials are required by law be properly abated prior to the demolition of
this structure.
The structure at this address, following proper abatement of the discovered
materials, is considered ready for demolition pursuant to the rules specified in
DEQ Division 340-248-0270 and LRAPA's Title 43-015-10-A&B regarding
asbestos.
All documentation from these activities and the credentials of those who
performed them are on file at the offices of Douglas International Inc.

Re: Pre-demolition asbestos abatement of home, attached carport and storage

On February 14, 2004 and April 2,2004, inspections and site surveys for
asbestos containing building materials were conducted at the above-mentioned

sampled. These asbestos materials are required by law be properly abated prior
to the demolition of this structure.
On April 12, 2004 the asbestos containing building materials found in this
investigation were properly abated by the Pacific Environmental Group Inc. of
Eugene Oregon. The waste generated is scheduled to be disposed of at the
Lane County Solid Waste Disposal Facilities Asbestos Disposal Site.
The house, attached carport and adjacent storage buildings, following this
exercise, are considered ready for demolition pursuant to the rules specified in
DEQ Division 340-248-0270 and LRAPA's Title 43-015-10-A&B regarding
asbestos.
All documentation from these activities and the credentials of those who
