This notice is intended to inform the Project Sponsor) Architect and Licensing Agency that the
above. noted project has been granted approval for occupancy and resident or pattent use by.
Facilities Planning and Saftty. Further agency comments) if any) regarding the project
completion are noted below.
Assisted Living and Residential Care Facilities require licensure by Seniors and
People with Disabilities (SPD) prior to occupancy by residents. New Ambulatory
Sur;gery Centers and new Hospttals require licensure by Licensing and Ce-rtification)
Oregon Health Services (OHS) prior to occupancy by patients. '
Information on licensure issues can be obtained by contacting Health Services) Licensing and
Certification) or Seniors and People with Disabilities Office of Licensing and Quality of Care.
NOTE:
Project approval is not an approval of omissions or oversights by this agency or of non-compliance
with any applicable Rules) Regulations or Codes. .' .
AGENCY COMMENTS:.
Project is approved contingent upon the following items:
1. Provide copy of balancing report. '
2. Provide copy of certificate of occupancy.
3. Complete installation of doors and frames on 1 st and 2nd floors. Expected completion date 4-
10-07

6. Provide separate room heat source for Common Bath 237.
7. Provide individual room identification for each resident room. Expected to be completed by
