A+A approved on 5/25/06. Requested and conducted field inspection of in-progress installation of 2 xerexes 40,000 gallon storage tanks at the CUP facility. Pea gravel backfill work was in progress. West tank anchor attachments to the deadman were below gravel backfill level and not accessible for inspection. The tank strap locations in the deferred submittal did not match strap locations as installed in the field. Asked Turner/Harder to submit new skectch that was in Harders possession and used to layout anchor bolt locations. This new skectch from Xerxes, titled Sacred Heart Hospital, #627-581, dated 2/1 7/06 matches the strap locations a sthey are int eh field and differ from manufacturers installation manual and operating guidelines booklet used as information for this submittal. This new sketch is submitted to be part of submittal #0575-15000 submission. Testing per manufacturers spec section 3.4 was discussed with contractor. Contractor to provide test results not witnessed by City Inspector. Confirmed with Springfield Fire Marshal taht it would be acceptable to allow continuing backfilIing with the understanding that the tanks would be retested adn witnessed after completion of backfilIing and before product pipe is connected. Confirmed with Steve Spitzer that backfilling and tank installation would be at risk until complete deferred submittal is received, reviewed, and approved by Anshen and City of Springfield. CCI Report #B-799 Fuel Tank Inspection - Backfilling at fuel oil tanks was put off and contractor repeated air test, listed in report #B-789 as not being witnessed by City Inspector. West tank pressurized to 4 psi at 8:00am, was 3.85 psi at 10:00am. East tank pressurized to 4 psi at 10:45 am, was 3.75 at 12:00pm. Fittings soaped at top of tanks, no leaks found. Hot air from compressor cooled in tanks account for pressure lost. Continued inspections required. Pressure test immediately after backfilling not needed. At the time of the pour the special inspector informed Hyland, and a call was placed to KPFF for a resolution. Gary with Hyland received a verbal answer that the reinforcing as placed would be satisfactory and confirmation would be forth coming. Observed this portion of system being prepared for inspection and test. Conditionally approved pending future required inspections and tests. CCI Report #P-929 2 portions of combination waste and vent piping are observed with a 10' static head going west to the horizontal building drain. All trap arms, floor drains, trench drains, and attachments will meet all installation and sizing requirements contained in Chapter 9 of the OPSC Table 7-4 for trap loading. The design professional shall submit signed and stamped calculations for resizing of floor drains, trench drains adn other connections before final approval for cover can be granted and concrete poured in this area. CCI Report #P-938 8" ductile fire feed pipe is observed installed in nw section of CUP. Thrust block is observed on 90 degree fitting on horizontal to vertical section coming up within bldg. All underground piping for fire mains shall be installed, anchored, flushed, and hydrostatically pressure tested before final approval will be granted (spec section 152802.3 D).
