NOTE: Before the Director can approve a DWP Overlay District Development request,
information submitted by the applicant must adequately support the request. All of the
Standards for Hazardous Materials within TOTZ must be addressed. Incomplete
applications, as well as insufficient or unclear data, will delay the application review
process and may result in denial. In certain circumstances, it is advisable to hire a
professional planner or land use attorney to prepare the required findings.

grantee of the following described real property, which was owned by said decedent at the time of death, without
probate of the estate, and I hereby agree to indemnify and hold harmless said Cascade Title Company from any loss
or damage it may sustain by reason of issuing its said title insurance policy without probate of said estate.

FOR TODD'S AUTO BODY, INC.

TODD'S Auto Body, Inc. is located in the 10-20 yr,time-of-travel zone to one of Springfield's municipal
drinking water wells. This means that chemicals that spill or leak onto the ground surface have the
potential to contaminate the well in 10-20 years. TODD'S Auto Body, Inc. is a committed partner in the
effort to protect Springfield's drinking water supply. Our hazardous materials management program
emphasizes the importance of preventing chemicals leaks and spills and responding promptly and
effectively should a release occur. We strive to increases awareness that the property lies within a
sensitive drinking water protection area among both our employees and the public who visit our
business.
This hazardous materials management plan outlines the protocol for TODD'S Auto Body, Inc. for
preventing, controlling, collecting, and disposing of any unauthorized releases of hazardous materials.
The plan also identifies the greatest areas of risk within our facility and outlines the steps we take to
reduce the risk. Any questions about this plan or how TODD'S Auto Body, Inc. manages its hazardous

Part B -- General Facility Description/Site Plan
In front of the building is paved parking for up to 8 vehicles.The building is a concrete block building
with cement floors.The office space is located up front with a restroom.
Inside the shop work space, is found a secondary containment storage cabinet,AREA#1 (see below on
facility map in yellow).The spray booth sits in the back, middle of the work space. Up to 10 vehicles can
be parked inside the building on either side of the paint booth. There's also a restroom and lunch room.
Outside the building, on the back of the lot is more parking and an alley (some paved, most gravel). There
is also two storage containers located in this back part of the lot.The first storage container, (8' x 30'), is
used to store all our metal fabrication equipment and tools.
The second storage container, (8' x 14'), is the secondary containment storage for paint products and
other chemicals,AREA #2 (see below on facility map in green). This second container is also covered
with a stick built roof, (approx. 17'x 17',shown below on facility map),that creates an overhang for
protecting another secondary containment,AREA#3 (see below on facility map in blue).

Section V: Record Keeping (Inspection and Maintenance)
Description of our Containment Inspection Program:
• Monthly inspections will be conducted and recorded on a "Containment Inspection Log"
that is on a clipboard located in the front office. (See example of that log included with this
report).
• The inspections will be visual, and will be conducted by Todd Fortier or Joshua Fortier,
both being authorized/responsible to sign off on the Inspection Log.
• The completed Inspection Forms will be kept in our records for 3 or more years.
If any problems are detected,they will immediately be reported to Todd Fortier. Todd will take
appropriate measures to resolve any problems. If Todd is unavailable for immediate response,Joshua
Fortier will take appropriate measures to resolve the problems.

Section VI: Spill Prevention
All new employees are trained immediately after hiring, about the importance of safety, the
hazards of chemicals and the prevention of chemical spills. Once a year, in January, all employees
are re-instructed. If a new hazardous chemical is introduced into the shop, all employees be
immediately instructed and educated with that product.
Measures being taken to prevent spills:
• All materials are stored at eye level or below.
• All shelves and cabinets are maintained to be safe and secure.
• Secondary containment is being used in three designated areas according to our site
& floor plan.
• Emergency spill kits are in place, at AREA #1 and AREA #2. The spill kit in Area #2

Section X: Notes
• A Wellhead Protection Sign is installed on-site to inform staff and visitors of the importance of
reporting and cleaning up spills. This sign is located on the outside, front of the building. Damaged or
missing signs will be replaced.
• Vehicle Washing Policy:
To keep car wash water out of the storm drain,all cars being washed behind the shop will be done
over gravel with biodegradable soap. Intermittently,smaller cars will be washed inside the shop,and
is advised whenever possible.
