I ORe: REPORT PUBLIC WORKS DEPARTMENT I CITY OF SPRINGFIELD, OREGON I I I I I I DISCRETIONARY USE APPLICATION ZON2004-00006 Planner: James Donovan, Planner II I The above application was reviewed by Public Works staff for pre-submittal requirements. The application, as submitted, does not include sufficient information for, staff to provide a formal plan review. The applicant will be required to resubmit plans addressing the following items: .- i 1. A Land and Drainage Alteration Permit (LDAP) will be required for this project. Plans for the LDAP must include all provisions for erosion, sedin\ent, and dust control. Truck tire wash and spill control measures will need to be addressed under this application. - 2. Additional detailed plans will need to be SUb~itted clearly indicating overall quarry operation that . includes types and locations of processing, truck routes on site and offsite, electrical and water source locations, dewatering / drainage locations, - . - I 3. Submit plans and documentation clearly outlining how the proposed quarry operation will meet and/or exceed the minimum requirements fo'r embankment and fill material in public right-of-way in compliance with the City of Springfield Standard Specifications Sections 302, 303, and 305. I 4. Submit proposed testing and inspection agehcy for this project along with firm's resume and qualifications. A full time inspector will be r~quired for the quality control of proposed fill and aggregate materials to be used within public: right-of-way. . 5. A "1200A" permit from DEQ appears to be ~ecessary for this proposed quarry operation. I . 6. Submit all applicable permits and approvals :from other outside agencies including but not limited to DEQ, DSL, Army Corps of Engineers, and all other county, state, and/or federal agencies as applicable. I 7. 'Submit an approval letter from the ParkslDe'parlment for the proposed quarry project, specifically addressing the quarry project impacts and fihal as-built condition as it relates to the final park plans and layout I i 8.
