* A site visit was performed at the above location to inspect the gravity retaining wall that has
. been installed at the back property line. A review of pictures taken at the site during installation,
and the working detail used was performed by this office. This criteri~ was. used to check the
design used by'using this data in the Anchor Wall Systems design prognim. This working detail
-is satisfactory for the installed wall. I was informed thaUhis-wallrequiredrepair. A review of the
picture showing the fail~e and inspection of the site conditions, indicates that the failure of the
wall was due to undermining the base of the wall at the adjoining property. The repair included
embedding 2 courses below grade to ensure the base of the wall and leveling pad was replaced on

solid, undisturbed native grade. It is important that these types of walls are placed on solid native
grade and are not undermined. Undet.minmg the base.ofthe wall will cause the, sagging failure
that is shown on the pictures taken oftthe failure. Attached is the working detail used for the
installation of the wall. The wall apPfars to be perfonning well at this time.
