and pursuant to the.fo!!owing tenns:
1. All plans and specifications referenced above are a part of this agreement.
,2. All proposed wor1l to be completed In a wor1lmanllke manner.
3. Any changes to the wor1l will be executed only upon written change order setting forth change in specifications, time
for completion, and contrect price. If contract price cannot be detennined, price to be time and materials, plus 15%
contractor's fee.
4. Owner to carry fire and casualty Insurance upon above described wor1l unless otherwise agreed.
5, Any sums not paid within ten (10) days of due date shall be subject to a finance charge of 1 1/2% per month (18%
APR) until paid. In the event of default by Owner/Contractor, Burrell Bros. Electric may immediately cease further
wor1l. In the event of suit or arbitration, prevailing party shall be entitled to all legal costs and attorney fees.
6. This proposal Is in effect for thirty (30) days at which time It shall automatically be withdrawn unless otherwise agreed.
7. If job is a residential contruction project, Burrell Bros. Electric Is the sole col1tractor on the job site, and price of the
proposed bid/project Is over $2,000 the Home OWner Is required to sign this bid agreement before wor1l will commence
and by signing this written agreement you are also acknowldeging you have received the 'Required Notices for
Residential C,onstructlon projects."
