Dear Mr. & Mrs. Crowson:
On January 5, 2010 the City of Springfield issued a decision for approval with conditions
of a Temporary Medical Hardship at 1434 D Street. The decision was valid for 12 months
and a 6 month extension was approved on January 5, 2011 due to expire on July 5, 2011.
A second six month extension may be approved if a written request is received prior to
the expiration date together with current written verification from a licensed physician
stating that the person requiring care continue to need care. The temporary living
quarters must continue to be in compliance with the initials conditions of approval for
Emergency Medical Hardship Case DRC2009-00044.
If the Medical Hardship no longer exists and/or the above information is not submitted to
the City by July 5, 2011, the temporary living quarters shall be removed within 30
calendar days.
