Dear Mr. & Mrs. Crowson:
I have received your request for an additional 6 month extension as required by the
Springfield Development Code. An updated written verification from a licensed
physician was also submitted.
A 6 month extension is granted until July 5, 2011. A second six month extension may be
approved if a written request is received prior to the expiration date together with current
. written verification from a licensed physician stating that the person requiring care
continue to need care. The temporary living quarters must continue to be in compliance
with the initials conditions of approval for Emergency Medical Hardship Case DRC2009-
00044.
