Applicant hereby agrees to install tile requested driveway to City of Springf.eld standards. The applicant further
agrees to have 600 of concrete in the sidewalk area (adjacent to Ole drivewayr.'and to pave the area behind Ule
back edge of the sidewalk, or drivemy apron, with a minim~m depth of J" asphattic concrete or 6" of portland
cement concrete. The area behind the sidev.'CJlk shaD be paved ill minimum or 18 feeL
The applicant iIIgrees that if he/she does not pave the a~a behind the sidewalk within JO days of cutting the
curb opening, the City of ,Springfield has the authority to close the drive\'l.'3Y access by removal of the curb cut.
An incurred costs shall be assumed by the applicant and if unpaid, said cost shall become a lien of the property.
When this application is approved by the City, the applicant must obtain a CtJrbcuUdriveway permil from the
Public Works Department, Engineering Division.
