This letter is written to verifY the street address of the hospital which will be located on
Lane County Reference Number 17032200, Tax Lot 00902. The hospital will be
commonly known as 3333 RiverBend Drive, Springfield, Oregon. The OHVI building
which is attached to the south of the main hospital will be commonly known as 3311
RiverBend Drive, Springfield, Oregon.
