made at the proper time. To request an inspection, call 726-3769
(recorder), state your City designated job number, job address, type of
inspection requested and when you will be ready for inspection. Requests
received before 7:00 a.m. will be made the same working day, requests made after
7:00 a.m will be made the following work day.

NO OCCUPANCY will be made of any structure without permission of the
Community Services Division, Building Safety. I further certify that only
contractors and employees who are in compliance with ORS 701.055 will be
used on this project.
I further agree to ensure that all required inspections are requested at the
proper time, that project address is readable from the street, that the
permit card is located at the front of the property, and the approved set
of plans will remain on the sitej1t all times during construction.

We, the undersigned property owners, requcst petmission to connect the following descl:ibed
property to the sanitary sewer owned and maintained: by the City of Springfield, hereinafter referred, as the
City. We thcrefore'agree topay a charge of$0.28 per squareJoot of the benefitting property for the first 150
;- feet of depth as a dep'osit against future assessments for sanitary sewer. Area in excess of the above
