Dear Mark:
I have reviewed the DWP application exemption request submitted by Bill Seider, dated
January 11, 2010, on behalf of Lane County Riverstone Clinic. The hazardous material
. inventory included with the original exemption request was incomplete. On July 7, 2010,
Jeri Weeks of Lane County submitted a revised inventory via e-mail (see attached).
The hazardous materials list submitted by Jeri Weeks indicates that the total quantity of
hazardous materials will not exceed twenty gallons and none of the products contain
DNAPLs. Based on the information provided in the inventory, the exemption can be
granted.
Please note that the facility may be inspected at any time to verify the information provided
in the exemption certification. If at any point the conditions of exemption are no longer
met, a Drinking Water Protection Overlay District application may be required.
