"E" Street. Svringfield. Oregon - Job #kv11035 As you requested, an inspection was performed on April 25, 2011, at the residence located at 3706 S. "E" Street in Springfield. The purpose of the inspection was to examine damage caused by a vehicle hitting the south exterior dining room wall of the residence. The vehicle struck the wall just below the dining room window. The structure is a single-story wood framed duplex. The unit that was hit is on the west half of the building. The exterior walls are 2x4 studs at 16" on center with 3/8" plywood sheathing and lap siding on the exterior, and \12" gypsum wallboard with a thin-coat plaster finish on the interior. See pictures 1-8 on sheets 3 thru 6 of this report to view the general vicinity and extent of damage done by the vehicle striking the south exterior wall of the duplex. At the time of the inspection performed on April 25, 2011 the following observations were made. All of the interior plaster finishes were removed from the south wall of the dining room, and the exterior portion of the damaged waIl was covered with a sheet of plywood (picture I). The impact of the vehicle hitting the wall displaced the wall into the residence (picture 2). At least four cripple studs below the window, the trimmer and king stud at the west side of the window opening, and an additional stud to the west of the window opening were separated from the wall bottom plate and pushed north into the dining room (picture 3). One of the cripple studs below the window was displaced 9\12" off of the existing wall bottom plate. The bottom of the trimmer at the west side of the window opening was split (picture 6), while the top of the king stud next to the trimmer was split (picture 7). The bottom of the west end of the window header was slightly twisted into the room. The 2x4 window sill was split in half, and the trim around the window was displaced and separated (picture 5). The plywood sheathing and lap siding below and to the west of the window were destroyed (pictures 4 and 8). The wall top and bottom plates appeared to be in good condition. The entry door to the west of the dining room was hard to open and close at the time of the inspection. A crack was visible in the concrete stemwall below the dining room window, but based on the dirt, spider webs, and discoloration within the crack it appeared to be pre-existing. The following repairs should be made to the apartment. All of the undamaged displaced wall framing should be moved back to its original position. Any 2x4 framing member that is split is to be removed and replaced. The displaced and replacement 2x4 studs are to be attached to the existing wall top and bottom plates with a Simpson A34 framing angle at each location. Verify that the window header is in good condition and return it to its original position. Verify that the exterior and interior walls in the area of impact are plumb and square. Replace the damaged exterior plywood wall sheathing, feltlhouse wrap, and lap siding to match existing. A minimum of3/8" COX plywood sheathing should be installed with 8d nails at 4" on center edge and 12" on center field. Block the panel edges of the new exterior sheathing. All fasteners in contact with pressured treated materials are to be hot-dip galvanized or equivalent. Install new R-15 high-density batt insulation in the empty 2x4 exterior wall cavities. Replace all damaged y," gypsum wallboard and thin-coat plaster finish to match existing construction. Verify that the window and door in the area of impact are in good condition and still function properly. If not, repair or replace as required. All repairs to the duplex are to be in compliance with the 2008 Oregon Residential Specialty Code as adopted by the City of Springfield. See sheets 7 and 8 of this report for the approximate location and description of the repairs that should be made to the damaged portion of the duplex.
