Delatte 10 No.: 1080867-88 Ice Inspection of: Epoxy Anchors Weather: Overcast' Continuous Inspection: 0 Contractor: Triad Mechanical BPNo.: SPR 2010-00041 Periodic Inspection: 0 Performed continuous inspection during the installation of two hundred twenty-three '(223) 518' stainless steel hooked all-lhread into existing concrete for gullet wall extensions as follows: filter basins N5 (82) and N6 (82); filler basin N4 (59). Prior to instaliation, the predrilled holes were cleaned with a brush and compressed air. Verified minimum embedment depth of 5'. Anchors were secured with Hilti RE 500SD epoxy adhesive (ICe - ESR 2322/ expiration date 08/2011). To the best of our knowledge, the work inspected was in accordance with plans dated 0812010 and applicable workmanship provisions of the I.B.C., except if noted above. . Reviewed By: \ ".
