Applicant hereby egrees to InstaD the requested driveway to City of Springf..1d standards. The appDcant further
agrees to have 6" of concrete In the sidewalk area (adjacent to the driveway). and to pave the area behind the
back edge of the sidewalk, or driveway apron. with a minimum depth 01 3" asphaltic concrete or 6" of portland
cement concrete. The area behind the sidewalk shaD be paved a minimum of 18 feel
The app6cant agrees that ~ he/she does not pave the area behind the sidewalk within 30 days of cutting the
curb opening, the City of Springfield has the authority to close the driveway access by removal of the curb cut
AD Incurred costs shaD be assumed by the applicant and ~ unpaid, said cost shaD become a lien of the property.
When this appHcation is approved by the City, the appHcant must obtain a curbcuVdriveway permitlrom the
PubUc Works Department, Engineering Division.
