Your Application for State Highway Approach Application number 11137, is
conditionally approved subject to receipt of the following materials:
~ A copy of the final local land use approval, including the approved site plan and
project description, that demonstrate that the locally approved plans and project
description are consistent with the plans submitted:1

Your subsequent Construction Permit and Permit to Operate, Maintain and Use an
Approach will be subject to the following terms and conditions as required under OAR
Chapter 734 Division 51:2
1. Applicant shall remove western approach at MP 2.83; Station 90+46, replacing
section with new sidewalk constructed and inspected through an appropriate permit
purchased from the City of Springfield by the applicant.
Note: In the event the approach 'ocations vary en the site plans, it may be necessary to reapply or request
approval of a modified site plan. A copy ofthis ietter has been sent to the local land development office.
Coordination with the local government and specifically, the local land use decision is set forth in OAR 734-
051-0070(10). .

This conditional approval expires 60 days from the date of this notice, unless both you
and the Department agree to an extension. Please provide the materials listed above by
5/24/2010.
PLEASE NOTE
THIS IS NOT A PERMIT TO CONSTRUCT AN APPROACH TO A STATE HIGHWAY.
Upon receipt of the materials listed above, the District Office will contact you regarding
submittal of approach construction drawings required. If your approach conforms to the
designs available in ODOT's Highway Design Manual and Standard Drawings, the
Department may be able to provide construction drawings for you.
