FINAL SITE PLAN REVIEW MINOR MODIFICATION 1198 Mohawk Boulevard Pacific Cascade Federal Credit Union DRC 2010-00016 The Conditions of Approval, as listed in the July 15, 2010 Staff Report and Notice of Decision, have been addressed as follows: Condition of Approval # 1: The submitted modification shall be revised to include a construction note or a revised drawing specifying that the base of the bio-swale shall be constructed to include medium comprising a mixture of sand, native soil, loam, and compost. The fraction of organic carbon (foe) in the constructed medium must be a minimum of 0.20, i.e., 40-50% organic matter. If the growing medium does not conform to this minimum foe requirement, the constructed medium shall be increased from the required 12-inch depth to a depth of 24-inches. The construction speciHcation has been added as Note 1 on Sheet SR3 (dated 9/2/2010), Condition of Approval #2: A wellhead protection area sign shall be purchased from SUB and installed in the parking lot near the bio-swale. The wellhead protection sign has been added to the drawing legend and shown on the drawing near the bio-swale aiea drain on Sheet SR3 (dated 9/2/2010), Condition of Approval #3: The submitted site plan modification shall be redrawn to close the existing 66-foot curb cut along Market Street and a curb-side sidewalk shall be installed in to match the connecting sidewalks Such amendments may be reviewed as a Type 1 ministerial action as allowed in SDC Section 5.17-145 (C). The changes in curb cuts and sidewalk have been added as Notes 9 and 11 on Sheet SRI (dated 9/2/2010). Condition of Approval #4: The submitted site plan modification shall be redrawn to close the Centennial Blvd. access. Access shall be restricted to that already provided by Market and Parker Streets and the exit-only access on Mohawk Blvd. An irrevocable Joint Use Access Agreement, approved by the City Attorney, guaranteeing joint use of the driveway access points by Lots 1, 2, 3, 4, and 5, shall be prepared and recorded. The changes to the curb cut has been added as Note 14 on Sheet SRI (dated 9/2/2010). Per 9/3/2010 correspondence from Mark Metzger, Planner III - City of Springfield, the Joint Use Access Agreement has been dropped as a Condition of Approval. Condition 01 Approval #5: Prior to approval 01 the final site plan, the applicent shall provide an operations and maint~nance plan'to the City lor review to ensure the long-term maintenance and operation 01 the proposed rain garden bio-swale. The plan should designate maintenance responsibility lor operating and maintaining the system, and should be distributed to all property owners and tenants 01 tj,,, site. The Operations and Maintenance Plan for the rain garden and cover letter from Branch Engineering, dated 9/2/2010, is attached. Condition 01 Approval #6: To ensure a lully lunctioning water quality system and meet objectives 01 Springlield's MS4 permit, the Springlield Development Code and the EDSPM, the proposed private vegetative water qualil~' swale shall be shall be Fully vegetated with all vegetation species established priol to approval 01 Final Site Plan. Alternatively, ilthis condition cannot be met, the applicant shall provide and maintain additional interim erosion control/water quality meaSUies acceptable to the Public Works Department that will suflice until such time as the swak vegetation becomes fully established. The SpringHeld Development Code objective has been added as Note 2 on Sheet SR3 (dated 9/2/2010). Five copies of the Final Site Plan Review - Minor ModiHc:ation'drawings (Sheet' SR1, SR2, SR3 and SR4) and the Operations and Maintenance Plan for the rain garden have been submitted with this Application for your review. Neither an application form nor an application fee are required for Final Site Plan Review. Minor ModiHcation.
