HAZARDOUS MATERIALS MANAGEMENT PLAN
(February 17,2010 - TO BE UPDATED ANNUALLY)
Welby's Collision Service is located at 1820 main Street, Springfield, in the 10 to 20 year time-
of-travel zone to one of Springfield's municipal drinking water wells. This means that
chemicals that spill or leak onto the ground surface have the potential to contaminate the well in
less than a year's time. Welby's Collision Serviceis a committed partner in the effort to protect
Springfield's drinking water supply. Our hazardous materials management program emphasizes
the importance of preventing chemicals leaks and spills and responding promptly and effectively
should a release occur. We strive to increases awareness that the property lies within a sensitive
drinking water protection area among both our employees and the public who visit our business.
This hazardous materials management plan outlines Welby's Collision Service's protocol for
preventing, controlling, collecting, and disposing of any unauthorized releases of hazardous
materials. The plan also identifies
the greatest areas of risk within our facility and outlines the steps we take to reduce the risk. Any

If hazardous, material inventory changes. this HMIS shall be updated to
reflect those changes and provided to SUB for review for compliance
with SDC 3.3-200 on an annual basis.
Products containing DNAPLS are prohibited.
Any new materials proposed for use or storage on-site shall be
evaluated for DNAPLS by the Springfield Utility Board Water Quality
Protection Coordinator (541-744-3745) prior to purchase.

SECTION V: RECORD KEEPiNG (INSPECTION AND lvlAINTENANCE)
Description of our Inspection Program:
The owner will conducte weekly inspections, and record completions, storing the HMMP forms
as ordinary business records for a period not to exceed 7 years.

inspected for mechanical damage, corrosion, and leakage
The facility floor is constructed of cast-in-place concr~te, and floor drains are connected to the
sanitary sewer system
If problems are detected with hazardous materials containers, and licensed waste hauler will be
contacte for removal. If problems are detected with the paint storage room (secondary
containment) the vendor of this equipment will be contected for repairs and/or replacments
SECTION VI: SPILL PREVENTION
To prevent spills, storage containers wiII be kept on purpose-built racks and secured against
falling or displacment. Large containers and paint equipment is hard-piped and is naturally
secured against spillage. Paint-room accidents, overspray, leaks will evaparate, or if the quantities
are too large, will be mopped up and secured in the paint storage area until they can be returned to
the vendor.

* The standard procedure for notification of a hazardous material release in Springfield is to call
911. The first responder will dispatch information t6 Springfield Utility Board, which must be
notified as soon as possible if a chemical release has the potential to contaminate municipal
wells.
