I have structured this letter stating your comments first, followed by my response in bold italics. My response is as follows: SDC 3.3-240 states: "The Director may attach conditions of approval that will minimize negative impacts of regulated substances on groundwater and ensure that the facility or the proposed developments can fully meet the standards specified in Section 3.3-235. These conditions may include, but are not limited to: on-site monitoring wells, Wellhead Protection Area signs, special storm water facilities or other conditions to address specific risks associated with the proposed development." I. Section I (B) and Section VI of the HMMP shall be revised to indicate that a drain mat (rather than an "absorbent spill mat") will be kept on site to cover the storm water catch basin in case of a spill. (1.23 acres) site. The site consists of the main building, the dumpster enclosure with sanitary drain and roof, the packaged generator unit with fuel tank leak detector including weather and fence enclosure with sealed and curbed slab and drainage device,4 HV AC exterior pad mounted packaged air handling units, and 38 vehicular parking spaces. Please see the attached Appendix D for the Time of Travel Zone Map indicating the site location and the Site Plan sheet A1DO.1 for the layout of the site and location of the above components. A spill kit shall be installed near the generator and dumpster enclosures to remediate any generator or dumpster related spill or any miscellaneous parking lot spill that may be hazardous and a drain mat shall be kept onsite for use to cover the stormwater catch basin in case of a spill. Please see Appendix F for a diagram of the generator area and Appendix G for the catch basin filter device in the parking lot. Part en Facility Map/Hazardous Material Storage Locations The interior of the building is 9,028 square feet of usable interior space as an out-patient dialysis clinic and includes space for mechanical and general storage areas, including water treatment and dialysis solution delivery system, patient treatment areas, and general support areas such as offices, toilet rooms and the staff lounge. The Janitor's closet is located conveniently to the mechanical/storage area and patient treatment area as this is where spills most likely may occur. Please see the attached Appendix D for the Floor Plan sheet A200.1 for the layout of the rooms and locations of stored materials. Section VI: Spill Prevention .General building (interior): Fresenius Medical Care is committed to preventing spills of hazardous materials and shall follow measures to prevent spills including storing containers containing hazardous materials at eye-level or below, securing shelves to walls, segregating incompatible materials, using and monitoring secondary containment in form of plastic tubs and trays as well as sealed/continuous flooring, maintaining all containers and machinery in good condition, conducting regular employee training, storing all chemicals inside, and inspecting all equipment on a regular basis. Back-up Generator and enclosure area (exterior): The diesel fueled packaged engine generator has been designed with a secured fence enclosure, weather enclosure, fuel tank leak detector, sub.base containment tray, and a sealed and curbed concrete slab foundation below the fuel tank/above the slab. The sealed and curb slab area is equipped with a flood gate that shall be kept open to allow for stormwater drainage to the parking lot storm drain equipped with a Flo-guard filter insert. During filling of the fuel tank, the operator shall manually close the flood gate to ensure that any spills shall be contained in the curbed area. Proper clean-up shall occur immediately if any spillage occurs during filling. A drain mat shall be kept onsite for covering the stormwater catch basin in case of a spill. Before the flood gate is opened again after filling the tank, the operator shall visually inspect the sealed slab for any spilled material. Signage with spill prevention instruction shall be posted on the generator. (See Appendix F) Trash enclosure area (exterior): AAll trash shall be kept in the dumpsters/containers supplied by the contracted waste removal services. A .visual inspection of the area shall occur on a weekly basis to ensure that materials are not leaking from the containers and that the slab area is free of debris and residue. The sanitary drain shall also be visually inspected to be sure it is not obstructed. HVAC (exterior): The HV AC pad mounted packaged heating and cooling units shall be maintained by a mechanical contractor per the manufacturer's instructions.
