RE: 3771 Olympic Street, Springfield, Oregon- Epoxy Anchor Special Inspection - W.O, 18792
As requested, a special inspection of the Simpson Strong-Tie ET22 Epoxy and threaded rod
installation was performed on December 1, 2010, The installation was performed in compliance
with the manufacturer's instructions and the ICC Report. Twelve 5/8" diameter threaded rods
were installed in %" diameter holes. Each hole was 3 3/4" deep, The batch number of the epoxy
was ET22041V556U, and the expiration date was 03/31/11.
