Dear Andy,
As required for Final Site Plan submittal requirements, the following addresses
conditions of City approval of the Tentative Site Plan Review application (DRC2008-
00042) for the proposed Birth Center.
Condition #1: The Final Site Plan shall depict the location and
dimensions of all existing and proposed easements affecting the property.
Easement information shall be _ consistent among all Final Site Plan
sheets.
Response: There are no existing easements affecting the property. Consistent with the
above condition of approval, proposed easements (7-foot wide PUE across the northern
frontage of the property and a north-south PUE for electrical service) are shown on the
Final Site Plan (Sheet A2.1) and on the Utility Plan (Sheet C3.0).
Condition #2: The Final Site Plan shall depict the location of
underground electrical lines. and transformers required to serve the
development site.
Response: The Final Site Plan (Sheet A2.1) and Utility Plan (Sheet C3.0) show the
location of underground electrical lines and transformer/junction box serving the site, as
required by the above condition of approval.
Condition #3: Prior to approval of the Final Site Plan, the applicant shall
execute and record Public Utility Easements as may be necessary to
provide utility services to the site, and provide evidence thereof to the
City. The Public Utility Easement dimensions and configurations shall be
satisfactory to the City's Public Works Department.

Response: As required by this condition of approval, attached is a recorded PUB
(Reception No. 2008-052968) reflecting those providing public utilities as shown on the
accompanying plans.
Condition #4: Prior to approval of the Final Site Plan, the applicant shall
obtain an Underground Injection Control Permit from the Oregon
Department of Environmental Qualiry and provide evidence thereof to the
City. The on-site stormwater management system shall be constructed in
accordance with the Final Site Plan.
Response: Application for UTC permit was submitted to the Oregon DEQ last week.
Approval is pending. When UIC approval is received the City will be properly notified
so the Final Site Plan may be approved consistent with this condition.
Condition #5: Prior to approval of the Final Site Plan, the applicant shail
provide an operations and maintenance plan for the Stormfilter
Catch basin for review and acceptance by the City. The operations and
maintenance plan shall ensure the long-term operation of the Stormfilter
Catch basin consistent with the criteria established by Storm water
Management Inc. The operations and maintenance plan shall designate
responsibility for operating and maintaining the system. and shall be
distributed to the property owner and tenant(s) of the site.
Response: An operations and maintenance plan for the catchbasins accompanies this
submittal for City review and approval as required by this condition. PeaceHealth will be
responsible for ongoing O&M.
Condition #6: The Final Site Plan shall provide for suitable street trees
along the Deadmond Ferry Road frontage of the site. Where street tree
installation is not feasible, provision shall be made for future installation
of street trees through execution of an Improvement Agreement.
Response: As required by the above condition, street tree locations are shown on the
Landscape Plan (Sheet LA-I).
Condition #7: Prior to approval of the Final Site Plan, the applicant shail
execute an Improvement Agreement for Deadmond Ferry Road for street
trees and street lighting.
Response: The City will provide PeaceHealth with an Improvement Agreement for
execution and recordation prior to Final Site Plan approval as required aboveO R . d

COllditioll #8: The Final Site Plan shall indicate or describe the location
of the nearest fire hydrant serving the site.
Response: Consistent with this condition, the nearest existing fire hydrant, located
northeast of the site on the north side ofDeadmond Ferry Road, is depicted on the Final
Site Plan (Sheet A2.1).
COllditioll #9: At least three (3) feet of clear space shall be maintained
around the circumference of all fire hydrants and Fire Department
connections in accordance with SFC 508.5.5 and 912.3.
Response: There are no obstructions around the existing fire hydrant, so the above
condition is met.
COllditioll #10: The Final Site Plan shall provide for site landscaping
trees that are at least 2-inch caliper at the time of planting.
Response: As required above, street and canopy trees specified are a minimum 2"
caliper and reflected on Sheet LA-I, the site Landscape Plan.
COllditioll #11: The applicant's photometric plan shall be adjusted as
necessary to indicate the presence of a screening hedge along the eastern
property line that prevents light trespass onto the adjacent property.
Response: The Photometric Plan (Sheet E3) was revised as required by the above
condition of approval to account for the existing laurel hedge to remain.
COllditioll #12: In the event the screening hedge along the east property
line of the subject site is altered in height or removed, the applicant shall
replace and/or adjust the light fixtures in the parking lot as may be
necessary to prevent illumination levels from exceeding 1.0 lumen at the
east property line. The Final Site Plan shall contain a note of this condition
for future reference.
Response: As required by the above, a note was added to Sheet E3 as required, and
reflects that proposed lighting with the existing hedge will not exceed 1.0 lumens at the
property line.

Condition #13: Prior to approval of the Final Site Plan, the applicant
shall obtain a Drinking Water Protection approval pursuant to Planning
Action DRC 2008-00044.
Response: The City approved the Drinking Water Protection application (DRC2008-
00044), so this condition has been met.
Condition #14: The applicant shall be responsible for ensuring
appropriate precautions are observed during site construction to protect
groundwater and to prevent spills or leakage of materials into the
stormwater system. Wellhead protection signs shall be posted at
conspicuous locations to alert contractors, subcontractors, employees and
others to the importance of reporting and cleaning up any spills.
Additionally, DNAPL materials shall be prohibited on the site during
construction and operation..
Response: The above condition will be met through the construction process, and the
project's general contractor has been informed of the above condition language and
requirements.
Condition #15: Prior to approval of the Final Site Plan, the applicant
shall obtain a Floodplain Overlay District approval pursuant to Planning
Action SHR2008-00007.
Response: As required above, the City granted approval of the Floodplain Overlay
District application (SHR2008-00007).
Condition #16: The applicant shall provide and maintain adequate clear
vision triangles at the corners of the site driveway in accordance with
SDC 4.2-130 and Figure 4.2-A.
Response: Vision clearance triangles are shown on the site Landscape Plan, Sheet LA-I,
that comply with Code standards as required above.
Condition #17: A maximum of one driveway will be permitted to serve
this property. The Final Site Plan shall provide a note and plan detail
indicating that the northwest driveway approach shall be removed and
replaced with vertical face curb, gutter, sidewalk (if necessary) and
planter strip in accordance with City standards.

Response: The site Landscape Plan (Sheet LA-I) and Final Site Plan (Sheet Al.l) show
the single driveway on-site required by the above condition of approval. Remediation of
the removed driveway as required above is reflected on Sheet LA-I.
Condition #18: The property owner shall be responsible for ongoing and
perpetual maintenance of the private stormwater facilities on the site to
ensure they function as designed and intended. and to ensure protection of
groundwater resources. Annual maintenance records shall be kept by the
property owner and provided to the City for review upon reasonable
request - normally within five business days.
Response: As property owner, PeaceHealth will be responsible for ongoing maintenance
of the site and its stormwater facilities as required above. Maintenance records will be
maintained as required and kept available for City review.
Condition #19: Prior to approval of the Final Site Plan, the applicant
shall obtain approval of a Tree Felling Permit initiated by Planning
Action DRC2008-00043.
Response: Consistent with the above condition of approval, the City approved the
applicant's Tree Felling Permit submittal (DRC2008-00043).

The Stormwater System contains all runoff within the site, All runoff will be disposed of by infiltrating into the ground,

This runoff is collected in roof gutter downspouts and catch basins connected to an underground piped storm System, The
conveyance from the catch basin goes through a Contech StormFilter. '
INSPECTiON/MAINTENANCE SCHEDULE
The whole system shall be inspected and maintained. quarterty and within 48 hOurs after each major storm event For this
O&M plan, a major storm event is defined as approximately 2,9. inches of rain in 24 hours. All components otthe stomi
system as .described above must be inspected and maintained frequently or they Will cease to luncllon effectively. The facility
owner must keep a log, recording all inspection dates, observaffons, and maintenance activities. .
INSPECTION/MAiNTENANCE PROCEDURE
The following items shall be inspected and maintained as stated:
Piped Storm System from the upstream end where the roof downspouts connect to the end of the infiltration basin.
. Shall be inspected for clogging or leaks at all cleanouts and connections to structures.
. Debris/sediment found to clog the pipes should be removed, and disposed 01 in accordance with applicable federal and
state requirements, .. .

. Shall be inspected for obstructions or debris, which shall be removed upon discovery,

Spill Prevention measures shall be exercised when handling substances that can contaminate stormwater, Virtually all
Sites; including residential and commercial, present dangers from spills. Ail homes contain a wide variety of toxic materials
including gasoline for lawn mowers, antifreeze for cars, nail polish remover, pesticides, and cleaning aids that can adversely
affect groundwater if spilled. It is.imporiant to exercise caution when handling substances that can contaminate stormwater.

grated catch basin.
. Tlle CSSF cons.ists ~f a sumped inlet chamber and a cartridge chamber. Runoff enters the suinped inlet chamber either
by sheetnowfrom a paved surface orfrom an inlet pipe discharging directly to the unit vault. The inlet.chamber is

. Maintenance procedures for typical ca.tchbasins can be applied to the CatchSasin StormFilter (CSSF). The filter
cartridges. contained in the CSSF are easily removed and replaced during maintenance activities according to the
.following guidefines.' . .
1. Establish a safe working area as per.typical catch basin service activity.
2. Remove steelgrale and diamond plate coVet (weight" 100 lbs. Each).
3. Turn ca~ridge(s) counter'l'lockwise to disconnect from pipe manifold.
4. Remove 4: center cap from cartridge and replace Witillifling cap.
5. Remove cartridge(s) from catch basin by handorwilh vactor truCK boom.
6. Remove accumulated sediment via vactor tnJck (min. clearance 13' x 24').
7.. Remove accumuiated sediment from cartridge bay.(min clearance 9.25' c 11').
8 Rinse interior of both bays and vactor remaining water and sediment
9. install fresh. cartridge(s) Ihreading clockWise to pipe manifold.
10. Replace cover and grate. ,.
11. Return original. cartridges to Storrnwater Management for cleaning and media disposal.

Operations & Maintenance Plan
Soakage Trenches consist of drain rock and sand, and receive stormwater from roof downspouts
and/or area drains. There are various components within-the system - piping, catch basin and the
trench itself. The Conveyance Piping consists of an inlet pipe (downspout or area drain); anoullet
pipe located between the catch basin and the soakage trench, and a perforated pipe. located on top of
the aggregate bed of the soakage trench. The catch basinis a structure receiving runoff from an iniet.
pipe' and conveying it to the soakage trench. The basin serves as the pre-treatment system for the
soakage trench; 'removing sediments an'd other debris that can impact its proper functioning. All
facility components, vegetation, and source controls shall. be inspected for proper operati6ns and
structural stability. These inspections shall occur', at a minimum, quarterly for the first two years from
the date of installation, then two times per year afterwards, or within 48.hours after each major storm.
The facility own.er must keep a log, recording all inspecti6n dates, observations, and maintenance'
activities. The following items shall be inspected and maintained as stated:'
Soakage trench infiltration: If water is noticed on top' of the trench within 48
hours of a major storm, the soakage trench may be clogged.
. Check for debris/sediment accumulation, rake and remove and evaluate
upland causes (erosion, surface or roof debris, etc
. Assess the condition of the aggregate and the filter fabric in the trench. If
there'is sediment in the aggregate, excavate and replace.
. . If there is a tear in the filter fabric, reoair or reo lace.
Conveyance Piping: If water ponds over the trench formore than 48 hours
after a major storm and no other cause, if identified, it may be necessary to
remove the filter fabric to determine if the perforated pipe is clogged with
sediment or debris.
. Any debris or algae grqwth located on top of the soakage trench should
be removed and disposed of properly.
. If the pipinghas,s<;>ttled more than 1-inch, add fill materiaL Iflhere are
cracks or' rele'ases, replace or repair the pipe. If there are signs of erosion
around the pipe, this may be an indicatipn of water seeping due to a crack
or break. ' .
Spill Prevention: Virtually all sites, including residential and 'commercial,
present dangers from spills. All homes contain a wide vilriety of toxic materials
including gasoline for' lawn mowers, antifreeze for cars, nail polish remover,
, pesticides; and cleaning aids that can adversely affect groundWater if spilled. It
is important to exercise caution when handling substances that can contaminate
stormwater.'

The Roof Soakage Trenches. are designed to infiltrate stonnwaterroof runoff into the ground.. All building roofJunoff from
the PeaceHeallh Medical Group Nursing Midwifery Birth Center wi! De infiltrated within the site by two Roof Soakage
Trenches. One basin will be located at tile soutll side of the building and the second basin will be located to the west of the
Birth Center: Roof runoff is collected in building downspouts and conveyed to underground piping and 10 catch basins.
Stormwater drain outlet pipe conveys stormwater from the catch .basin to the roof infiltration basins. The roof infiltration
basin is a below ground gallery of perforated pipes placed within open graded rock trenches. . The two .roof infiltra~on basin
systems shall be inspected and cleaned a!lOually and within 48 hours after each major stonn event Sedimentation.
manholes rriust be inspected and maintained frequently or they will cease to function effecUvely. The facility owner must keep

storm events. During normal stonn events, ponding should not occur althe surface. Frequent ponding oi water within lhe
basin areas may indicate thallhe basin is failing due. to siltation, or the dogging of the pipe periorations. Clogged pedorated
pipes must De replaced.. .
Source C,ontrol measures prevents pollutants from mixing with slormwater. Typical non-structural control measures include
raking. and removing leaves, street sweeping, vacuum sweeping, limited and controlled application of pesticides and
fertilizers, and oth.er good house keeping practices. .
