Amy: Attached is a sample lease agreement prepared by the applicant for a proposed multi-tenant commercial building
at 1815 Main Street. One of the requirements of the site plan approval was preparation of lease agreement language
that satisfies DWP requirements. Please review and let me know if you have any comments or concerns that can be

Rick: Attached are newly updated forms tor dedication of a Public Utility Easement along the property frontage. Please
have the property owners sign and notarize the PUE dedication form prior to submittal with the Final Site Plan
application'packet. The legal description submitted previously can be appended to the easement form. Please let me
know if you have any questions.
Staff are reviewing the plans and respo'nses to conditions of approval you submitted last week. If I have any comments
to pass along from the staff review I will let you know prior to requesting submittal of the Final Site Plan application.

Subject: Chan Family LLC project PUE
Andy,
I checked over this PUE, and the information on the form and the legal description look fine. The only thing is that there
is a new PUE torm as we talked about. Attached is a PDF of the form and a Word tile which is a "fill in the boxes" form
and is locked so only the boxes can be edited (Public Utility Easement - Corporation). I went ahead and filled but the
new torm which you could provide them as well if you want. Since the Grantor is an LLC, it should be treated as a
corporation and the filled out form is also so we can check to see if they have the correct signees (both or either one or
the other "members") for the LLC if they do the form themselves.
