Fenstermacher 10 No.: 5258882-49 ICC Inspection of: Screw Anchors Weather: Inside Continuous Inspection: 0 Contractor: McKenzie Commercial BPNo.: COM 2010-00799 Periodic Inspection: 0 Performed continuous inspection during the installation of one-hundred sixty (160) 5/8" x 4" vertical Simpson Titen-HD screw anchors (ICC - ES ESR 1056) into concrete slab on grade for sill plate attachment at interior partition walls. The predrilled holes were drilled at least 1" deeper than required embedment depth of 2 3/4". Anchors were tightened to a snug-tight condition with a Milwaukee 3/4" impact driver. To the best of our knowledge, the work inspected was in accordance with RFI 3 dated 08-19-10, RFI 8 dated 09-14-10 and applicable workmanship provisions of the I.B.C., except if noted above. Currently, all weld booth walls are being constructed and are at varied heights ranging from :t3' to 8'. Installed rebar was checked for size, length, spacing, quantities, grade and laps, where applicable. Cells were relatively clean, and free of excessive mortar projections. Advised by Mr. Henderson that walls will be grouted solid in one (1) lift and City of Springfield inspection has waived requirements for clean-outs at base of reinforced cells. Masonry material testing requirements will be verified with design team. To the best of our knowledge, the work in-progress was in accordance with prints dated 06-21-10 and applicable workmanship provisions of the I. B. C., except if noted above. Delatte ID No.: 1 080867 ~84 ICC Inspection of: Masonry Weather: Inside Continuous Inspection: 0 Contractor: McKenzie Commercial BP No.: COM 2010-00799 Periodic Inspection: 0 Performed final inspection on the reinforcing steel in 8" CMU walls for new welding booths, door infill west of welding booth, infill in rooms 112 and 108A (slab to top of walls except for infills). Contractor stated last course of infills will be "slugged" with mortar. Reinforcing steel was checked for grade, size, lap, placement, quantities and clearance, where applicable. Reinforced cells were relatively clean and free of excessive mortar projections. . Observed the placement of 7.5 yards of Knife River 7.5sk grout (mix 247KG3R200) for . walls noted above. G'rout was placed by pump and was mechanically consolidated. Slump - 8"+; air temperature - 60F; grout temperature - 69F.
