Grace and Tim,
After speaking with Sylvia Rieger with the State, she would like to see Condition one of the MDS decision (building
permits) completed before she issues Condition two (the issuance of the certificate of registration for the Room &
Board). If the building permit review, issuance and inspections take longer than the 30 days listed in condition two, we
can look at extending the timeline.

Yes, that will work. I'll let Tim and Grace know that we'll require condition one to be met before condition two and
that we can extend the 30 day timeline if the building permits take longer to obtain. Does that sound OK? I can keep
you updated on the status of the building permits and inspections.

Condition 1 is the one I'm most interested in seeing her satisfy, and I'd prefer that be
done prior to the re-issuance of her certificate of registration. Will that work with your

Hi Sylvia,
Do you know if there are certain conditions you would like to be completed prior to issuance of the registration such as
the permitting for the areas of the building built without permits or would you be more comfortable with the other
conditions being completed before the registration was issued?
If the registration were issued and all other conditions were not met, is there a way to revoke the is ued r gistration or
is it valid for an entire year?

Helio Liz,
I was just reviewing the document you forwarded and I wanted to check in with you
before authorizing Ms. Almeida's certificate of registration for a room and board
facility. At the bottom of page 1 your recommendation is "approved with conditions."
Also, Condition #2 requires Ms. Almeida to submit a copy of the current room and
board registration.
Although she has been working to achieve compliance, Ms. Almeida's registration
lapsed due to noncompliance with the Springfield Development Code. Since she does
not have a current room & board registration at this time, she cannot satisfy Condition
#2.
My question for you Liz is about the status of this facility. I'm wondering if I can
reissue her room and board certificate at this time given she's "approved with
conditions" by your department.
