Dear Ms. Almeida:
I am a Policy Analyst with Seniors and People with Disabilities. I have been given
responsibility for overseeing the Room and Board program. I am reviewing the
pending applications for new registrations and renewal of existing ones. I am writing
to provide a status update on your 2009 application for renewal of your registration
for your facility at 275 S 70th in Springfield.
You submitted an application for renewal in August 2009. While that application was
pending, the City of Springfield determined that you exceeded their limit of 5 resident
bedrooms and that you had constructed 5 additional bedrooms without permits. You
submitted an amended application showing the use of 5 resident bedrooms and 1
caregiver bedroom. This configuration would be acceptable to the city.

However, their review identified 8 specific conditions that would have to be met 7-
before they would approve the smaller facility. This office understobd that you were " Y

registration. However, as of t-ebruary we were advised that you were consldenng )
applying for a license to operate either an adult foster home or a residential care /
facility on the site. .

I do not have any evidence to show that you have submitted either application or
have complied with the city's conditions. Please let me know what your plans are
and, if you want to resume operating a registered room and board facility, how and
when you will comply with the city's conditions.

( Pleased be advised that your registration has not been renewed and you are not
~ currently authorized to operate a room and board facility pursuant to the laws of the
l ~ity of Springfield. If you have residents in the home at this time, you must
immeCliately give them notice to move out. All residents must be relocated by July 1,
2010.
You are not .licensed as an adult foster home or residential care facility. If you do
apply to be licensed under one of these programs, you will have to wait until the
application process is complete and the license is issued before admitting any
residents.
I am looking forward to working with you if you choose to go forward with the smaller
room and board facility. Please feel free to contact me if you have any question or
need assistance with this process.
