Foundation requirements - new structures on residential lots
This form may be used as a temporary verification to allow construction to continue
on the job site until the consultant's stamped affidavit on the site soil compaction
and stabilization is submitted to the City. This form must be completed by a
licensed -design professional (engineer or architect) or his/her authorized employee,
and submitted to the building inspector prior to requesting City inspections or
placing foundation concrete. It is important that all questions be answered
completely for the foundation site to be approved for construction.

The geotechnical report for the Mountaingate Subdivision Phases I & II makes
recommends for placement of building constnlction on each site. The report also
recommends that site preparation during wet weather conditions be avoided unless
special measures are taken to mitigate soft soil conditions and moisture collecting in' or
around the foundation areas during and after the construction process. Positive site
drainage must be provided.

The undersigned hereby affirms that the excavation, structural fill and moisture
stabilization methods for the building site at the address shown above were observed by
me or an authorized employee of my firm and that the following is true:
1. The geotechnical information for the subdivision" specific to this site, was utilized
as part of the determination for foundation preparation and drainage requirements,
2. The foundation sub-grade is capable of supporting a minimum of 1500 psf, and is
adequate to support the building proposed for this site.
3. The moisture content of the excavation was adequately maintained during the site
preparation process and the site is adequately graded and drained for discharge to
an approved location in order to maintain stable moisture content throughout the
const:I1Iction process and thereafter.
4. The accompanying report titled "Site Investigation Questionnaire for Consulting
Design Professionals" containing field observations and instructions made on

As you requested, a site soils investigation was performed at the above address, Site visits were
performed on July 19,20,21,2005 to supervise the excavation of the site for the proposed
construction.
All unsuitable fills, organics and other unsuitable material has been removed to suitable grade,
The excavation required the removal ofunsuitabJe landslide and expansive soils to a depth of
approximately 6'.8' on the south portion of the site, to suitable decomposed sandstone. The
' sandstone has a Unified Classification ofML-SM and a plasticity index of less than 20. This soil
is not considered expansive (after Holtz 1959). Imported compacted crushed 3" open was placed
and compacted in lifts of 6'~_8" at the base of the excavation to within 24" of the desired footing
elevation for the placement of the stepped foundation walls. 1 W' minus crushed rock was
placed at the top 12" to 24" of the excavated site placed and compacted in 6"-8" lifts.
A compaction inspection on the gravel fill was performed on 07/25/05. The compacted surface
of the I W' minus crushed rock is adequate for the proposed foundation and has a minimum
bearing capacity of 1500 psf, The compaction of the gravel surface is greater than 95% of a
standard proctor per ASTM D698.
Perforated drainlines were placed at the Jow point of the excavation, as directed by this office,
and are to be tied to the approved outlet for this subdivision's storm water system. Drainlines as
specified on the site plan at the back of the lot are not feasible, due to the possible damage to
existing vegetation root system for vegetation that is to remain, The drain line has been installed
at the edge and base oft1~e engineered fill in open rock at the back of the structure excavation and
is to be tied to the approved outlet for storm water, This has been placed in addition to the
perimeter footing drains that are to be installed at the back of the structure as per plan.

It is recommended that the back patio is to be installed as a concrete slab that will be sloped to
provide that the runoffwi1l be directed to the back of the slab drainage systems as described
above. A pervious paver system for the slab is not recommended, as the footing drains are to the
back of the patio, and surface water from the patio would be allowed to build up against the
house foundation, which may cause damage to 'the structure foundation in this area.
This report is limited to the site soils investigation related to soils and fill in regard to soil
conditions at the excavated structure site at the above address only.
