Dear Ms. Boyd,
Enclosed is a receipt for the check you mailed for an electrical permit at the above
referenced address. However, when we received the form it was not filled out in its
entirety and the 5% State Surcharge and the 3% City Administrative Fee were not
included. Please send a cheek for the difference of $3.12 at your earliest convenience.
We have already validated the permit so this has not held up' the inspections on your
project. Thank you for your anticipated cooperation.
