1. I state that I am a Secretary for the Planning Division of the Development
Services Department, City of Springfield, Oregon.
2. I state that in my capacity as Secretary, I prepared and caused to be
mailed copie~ of Notice of Proposed Amendment to DLCD for
LRP2009-00013 Annecxation of a segment of public street ROW.
(See attachment "A") on October 29,2009 addressed to (see Attachment
"B"), by causing said letters to be placed in a U.S. mail box with postage

This form must be received bv DLCD at least 45 davs prior to the first evidentiarv hearin!!:
per ORS 197.610 and OAR Chapter 660. Division 18
1. This form must be submitted by local jurisdictions only (not by applicant).
2. When submitting, please print this form on light green paper.
3. Text: Submittal of a proposed amendment to the text of a comprehensive plan or land use
regulation must include the text of the amendment and any other'information the local
government believes is necessary to advise DLCDof the effect of the proposal. "Text" means the
specific language being added to or deleted from the acknowledged plan or land use regulations.
A general'description of the proposal is not adequate. Do not" submit this form without
supporting documentation.
4. Maps: Submittal of a proposed map amendmimtmust also include a map of the affected area
showing existing and proposed plan and zone designations. The map should be legible and on
8)1, x 11 inch paper. Please provide the specific location of property, such as an address and/or
tax lot number. Include text regarding background and/or the justification for the change, such as
the application accepted by the local government.
5. Exceptions: Submittal of proposed amendments that involve a goal exception must include the
proposed language of the exception.
6. Unless exempt by ORS 197.610(2). proposed amendments must be received at the DLCD's
Salem office at least 45 days before the first evidentiary hearing on the proposal. (The clock
begins on the day DLCD receives your proposal.) The first evidentiary hearing is usually the
first public hearing held by the jurisdiction's planning commission on the proposal.
7. If you have an electronic copy of the proposal, we would like you to submit one electronic copy
[email.CD. or upload to DLCD (for submittal instructions, see # 4)] and ONE PAPER COPY
of the proposed amendment to:

EXHIBIT A
A portion of the 19th Street right of way from the intersection with Hayden Bridge Road
Southerly; being more completely described as follows:
Beginning at the Initial Point of Land Partition Plat No. 92-P0306, as filed for public
record in the Office of the Lane County, Oregon Surveyor, in County Surveyor's File No.
31021, on January 4, 1993; thence along the following five (5) numbered courses:
1) North 000 03' 23" West a distance of 170.00 feet along the Westerly line of the
said partition plat to a point on the North right of way line of Hayden Bridge Road;
2) thence North 890 58' 29" East a distance of 35.00 feet along the Northerly right
of way of Hayden Bridge Road to a point;
3) thence leaving the North right of way line of Hayden Bridge Road, crossing the
road and along the Easterly right of way line of 19th Street, South 000 03' 23"
East a distance of 986.99 feet to a point of curvature in the easterly right of way;
4) thence leaving the Easterly right of way of 19th Street and crossing it at right
angles, South 890 56' 37" West a distance of 35.00 feet to a point on the
Westerly line of above said partition plat;
5) thence North 000 03' 23" West a distance of 817.01 feet along the Westerly line.
