DWP Site Visit
I inspected Riverbend Medical Pavilion today (5/5/1 0) with Tamara Miller and April Wyatt
(Facilities, Plant Tech, Maintenance; cell: 556-1946).
We toured all PeaceHealth facilities and Oregon Imaging. Walgreen's and the Willamette Valley
Cancer Institute have separate DWP permits and will require separate inspections.
Inspection Results:
. RMP has an updated HMMP and HMIS created by KTA. Tamara will send me electronic
copIes.
. Staff training: Tamara will look into whether or not the talking points I sent in September of
2009 have been included in annual staff training materials.
. Coastwide Laboratories will be installing new dilution centers in the janitor closets. Karl
Bridenbeck, the Coastwide rep with whom I had discussed the secondary containment
requirements, is now on personal leave. The new representative is Ross. I will speak to him
about secondary containment requirements and ensuring that chemicals don't drain into floor
drain.
. RMP has not been following the monthly inspection program outlined in the approved
HMMP. Tamara and April decided that, from here on out, April will conduct the monthly
inspections for all Peace Health spaces.
. Areas that need secondary containment:
o Throughout facility: cabinets under sinks.
o First floor:
. Urgent Care: Room #1-625, Lab, Soiled Utility Room
o Second floor:
. Endoscope Processing: alcohol, Cidex, detergent
. Endoscope procedure rooms (3): formalin, chemicals under sink
o Third floor:
. Neuro med closet
. Orthopedics med closet
o Fourth floor:
. Pediatrics med station
. Oregon Imaging had a few chemicals (paint thinner) in cabinets without secondary
containment. Overall quantities are small. Tamara will look into whether or not Oregon
Imaging should have a separate permit and whether or not they will qualifY for a DWP
exemption.
