Clair Company, Inc. (CLAIR) has reviewed the submitted documents for the Excavation and Grading Only
Permit for the Kohl's Department Store Project on behalf of the City of Springfield (City). CLAIR recommends
issuance of the above referenced permit, with the following Conditions of Approval as noted in the attached plan
review document.
CLAIR requests that the permit applicant/designer respond in writing to each comment in the Conditions of
Approval plan review document by adding an applicant response row directly below the CLAIR review
comment field and fill in the indicated fields on the attached EXCEL2000 document along with submitting all
necessary requested submittals. '

Submit City Special Inspection acknowledgement form for
approval by City Building Official. Special Inspection will be as
follows: soils grading, excavation and fill, and final foundation
preparation. OSSC 1704
Prior to placement of foundation concrete the complete
summary and Final Report, as observed by the Geotech of
Record, of the structural fill placement and compaction must be
submitted for review and approval per Design

EXCAVATION AND GRADING - ELECTRICAL PLAN REVIEVI.
Electrical conduit placed in or beneath the building slab, if any,
must be shown on the electrical prints and be presented for plan
review. The electrical prints dated 05/04/05 show no conduit in
or under the concrete slab within the footprint of the building.

be approved for their specific use and location relative to the
building footprint as per the provisions of Section 604.0 Any
existing pipe and fittings within the building footprint that are not
approved for use within or under the building shall be removed
