Dear Ms. Triplett:
This correspondence confirms that all items identified on the Project Inspection list for
the above-referenced freestanding birth center have been completed. Please forward
confirmation and written notification ofproject approval to me at the address below.
Specific response regarding resolution of each item identified on the Project Inspection
list by Mike McDermott is below or attached:
I. Complete sidewalks, curbs, parking lot & landscaping.
Driveway, parking lot, sidewalks, and curbing was completed by April 9, 2010. All
striping, required signage and ADA ramp'installations were completed as well by that
date.
2. Complete HVAC installation.
Heat pumps were iristalled March 19, completing the HV AC installation.
3. Complete misc. tub Plumbing & install ice machine.
Ice machine installation was completed on April 9, and final tub plumbing was
completed on March 19.
4. VerifY Architects Punch List is completed.
The punch list was identified by the Owner, and reviewed with the Contractor and
includes insignificant items, such as minor painting touch-ups. An items of any
significance have' been completed and the Owner has issued a Notice of Substantial
Completion to the Contractor. .

This notice is intended to inform the Project Sponsor, Architect and Licensing Agency
that the above building has been approved by Facilities Planning and Safety.
NOTE: Assisted Livin1; and Residential Care Facilities require licensure by Seniors and
-..- People with Disabilities (SPD) prior to occupancy by residents. New Ambulatory
Surgery Centers and new Hospitals require licensure by Health Care Regulation &
Quality Improvement, Orepon Health Services (OHS) prior to occupancy by patients.
Information on licensure Issues can be obtained by contacting Health Care .
Licensure & Certification, or Seniors and People with Disabilities Office of
Licensing and Quality of Care.
Project Building ARproval is not an approval of omissions or oversights by this agency or of
non-compliance with any applicable Rules, Regulations or Codes.
