This request is for Site pIan Review approval fora new ~ingle-story
outpatient/assessment building with associated site improvements to The Child
Center located at 3995 Marcola Road in Springfield, oregon. The proposed new
office building is about 5,151 square feet in size and will be located near the existing
office buildings on.the site. The Site Plan Review application includes information for
the entire development site, although the focus of the ~equest is for proposed
changes to the existing facility and grounds. The propo~ed new buildingwill shift
where services are provided on the site and improve th!'quality of office space,

children that will be served on The Child Center campus.
B~ Proiect Obiectives
Operating since 1971, The Child Center provides a unidue, valuable service to .
families in Lane County. The proposed site plan primarily addresses the fOllowing
objectives: .
1, Enhance thequalitY'of serVices by improving th~ office space used for
counseling and assessment activities.
2. Improve the safety of the site's access and on-s!tecirculation for,busses,
motorists, bicyclists and pedestrians.

In response to suggestions from EWEB and City of SpriQgfield staff, the project also
incorporates relocating the lower parking area to a higher elevation and greater
distance from the river, The existing lower parking are~ is very <;Iose to a major
source of drinking waterfor EWEB, The proposed proj~ct will retain the eXisting
unimproved gravel area for emergency vehicles but will! not be used on a regular,
daily basis by the Child Center. Relocating the parking area requires a substantial
capital investment by the Child Center and is dependenton"! successful fundraising
campaign, Relocating the parking area will reduce the potential negative
environmental impacts on water quality and riparian ve,getation. .

C. Overview of The Child Center Services
The Child Center is a community-based private non-profit agency that has served the
Lane County region since 1971. The Child Center provides a continuum of .
psychiatric and special education programs to children with severe emotional and
mental disorders. Day treatment services include therapeutic and' special education
programs that' pro\!i~e "khort-term, intensive, or school-based assistance" to children
ages 3 to ;12. Day treatnient services are'provided on aohe-to-one basis or in small
groups at The Child Center. The Child Center also provides individual or small group
counseling at other locations including the child's home or school. This community-
based out-patient program focuses on children betwee~ the ages of 3 and 18 with
emotional, behavioral and mental disorders. A crisis hotline is also maintained to
provide a 24-hour response to a mental health emergel1cythatincludes a child. This
hotline can. activate a mobile response team, a variety qf respite care options anda
referral system that helps the child receive appropriate care.

Over .the past few years; children are increasingly servei:! in area schools,
Approximately 24 staff provideservices while working at area school sites. This shift
in where services are provided has also resulted in a d~crease in the number of
school busses transporting children to The Child. Center;.
D. Overview of The Child Center Camous
The Child Center is located on a generally terraced site pn Marcola Road near the
Hayden Bridge Road intersection. The entire development site includes about 8.4
acres. The improved portion ofthecampusis'significaf)tly less and encompasses
about 4.2 acres.
The existing campus includes:
OutpatienVoffice building - 784 square feet (toibe demolished)
Older residence/office - 1,200 square feet .
Maintenance building - 1,200 square feet
Administration building -:- 4,717 square feet.
Classroom building - 2,479 square feet
Office/classroom building -3,611 square feet
Modular office/classroom - 1,850 square feet
Library - 1,200 square feet
Currently, there is a gravel parking lot at the lower plateau primarily for staff, visitor
parking spaces on the upper plateau, and overflow p'ar~ing spaces at the older
residence used for office space. . . ,
The property is surrounded by natural vegetation at the;property lines to the east and
west, the Mckenzie River to the south and Marcola Road to the north. The existing

Zoning: The site is zoned Pu.blic Land & Open Space. Property to the north is
zoned either HeaVy Industrial or Public Land & O'pen Space. A narrow strip of
land to the west, south of Marcola Road is zoned Heavy Industrial. Property to

III. BACKGROUND OF THE SITE
In March of 2004, The Child Center initiated a zone cha:nge application for the
subject property to bring the zoning into compliancewit,h the Metro Plan designation
and to make the uses consistent with the zoning. (Refe:r to City case no. ZON2004-
00005.) On May 18, 2004, the Springfield Planning Commission approved the
requested change in zoning from HI Heavy Industrial to PLO Public Land and Open
Spaca . "
According to the Final Order approved by the Springfield Planning Commission, the
zone change, would bring the property in conformity wit~ the Metro Plan designation
of Parks &Open Space, In addition, the PLO zoning "would permit private, not-for-
profit schools and treatment facilities, therefore alleviating the non-conforming
status", The Final Order acknowledged that following approval of the zone change,
The Child Center intended to submit a Site Plan Review!application for the expansion
of the existing treatment facil\ty. i
IV. SITE PLAN REViEw APPROVAL CRITERIA
The Director shall approve or approve with conditions: a Type /I Site Plan
Review application upon determining that approval criteria A. through Eo,
below have been satisfied. If conditionscannotjbe attaghed to satisfy the
approval criteria; the Director shall deny the app'lication. Decisions approving
an application, or approving with conditions shail be based on compliance
with the following criteria in Springfield Deveiopment Code Section 5.17-125:

applicable Refinement Plan diagram, Plan District map, and

The Metro Plan designates the subject site as Heavy Industrial. 'This land use
designation is intended to provide land for businesses that will help diverse
the economy'and thafoften have unique charac~eristiCs with special siting
requirements. To provide consistency between the former plan designation of
Parks and Open Space and local zoning, the Springfield Planning Commission
approved a request from The Child Center to ch~nge the zoning from HI Heavy
Industrial to PLO Public Land and Open Space. f,lccordingto Section 3.2-705,
the PLO, zone is established to prOVide land for goyernment and educational
uses. These include schools and treatment facilities.

There is no applicable City Refinement Plan,Pla\! District map, or Conceptual
Developm~nt Plan for The Child Center.' .

B. Capacity requirements of public and ptiv?te facilities, including but not
limited to, water and electricity; sanitary ~ewer and stormwater
management facilities; and streets and traffic safety controls shali not
be exceeded and the public improvements shall be available to serve
the site at the time of development, unle{;s otherwise provided for by
this code and otlJer applicable regulatio~s. The Public Works Director
or a utility provider shall determine capaCity issues.
The subject property is within the Springfield Urtian Growth Boundary and the
City limits. It is currently served by the key urba8 services as prescribed in the
Metro Plan except for stormWater facilities. Since there are no stormWater
facilities available to the site, the project will include retaining, treating, and
disposing of all.stormwateron site and thru the use of vegetated infiltration
basins (rain gardens).
C. The proposed development shall comply 'with all applicable public and
private design .arid construction standarqscontalned in this Code and
other applicable regulations,' .
The Child Center is a non-profit organization est~blished in 19T1.and
operating at the present site since 1974 when it purchased and occupied the
former Town and Country Day School. Followingthe initial land acquisition,
The Child Center purchased an adjoining lot to thesoutheast to expand the
facility to.serve the growing volume .of students fileeding treatment services.
Assessor!s Map 17-02-30-00, Lot f09remains Ondeveloped except for a
single-family,dwelling rented to staff of The Child Center. The proposed
changes to the site are limited to a small portion of the development site.
Proposed new site improvements comply with applicable standards as further
discussed below.
Section 4.2-105 Public Streets

The proposed development does not trigger the reed to create or extend any
new public streets. The.ChildCenter will continue to have direct accessto
Marcola Road, a minor arterial street. The section of Marcola Road adjacent
to The Child Center meets the standard street right-of-way and paving width
dimensions. No additional public right-of-way oristreet improvements are
needed. One existing curb cut/driveway on MarcoJa Road will be closed as
part of the project The closed curb cut/driveway will be replaced with a
s\andard curb, sidewalk and planting strip consistent with ttierest of the
street frontage abutting the property on Marcola, Road.

Currently there are two driveways on Marcola R~ad that serve The Child
Center c~ampus. Both of the existing driveways '1re located at the northeast
area of thecarnpus and each allow full. turning n)OVements. The centerline of
each driveway is within about 50 feet and the edge of the pavement areas are
within 30 feet. This driveway spacing is confusi~g to the motorist and does
not,meet the spacirig standards for safe turning;movements,' Furthermore,
the main driv'eway entrance has an unusual configuration immediately after'
entering the,site.
One existing driveway is on Assessor Map 17.02c20, Lot 700 and the second
is on Asse!3sor Map 17-02-30, Tax L:ot 100. There is currently no pUl:Jlic
access from Marcola Ro~d to Assessor Map 17-02-30, Tax Lot 200 located in
the,southern portion of the campus. There"is also no public access from
Marcola Road to Assessor Map 17-02-30, Lot 100.
The proposed development retains two driveways to the site but significantly

erihances,the safety of the access and on"site circulation by increasing the
distance between the two driveways and aligning-one driveway with an
existing one on the opposite side of Marcola Road. Although the existing
driveway to the lower plateau will be retained, it,willprimarily serve as an
additional access for emergency vehicles or occasional needs to do repair and
maintenance on the buildings or grounds, In gen. eral, the driveway to the

Foreclosure, Forfeiture, etc. Commencement of foreclosure or forfeiture proceedings, whether by judicial prcx:eeding, self-help, repossession or
any other method, by any creditor of Grantor or by any governmental agency against any of the Property. However, this subsectfon shalf not apply
in the event of a gaod'faith^dlspute by Grantor as to the validity or reasonableness 01 the claim which is the basis of the foreclosure or forefelture
proceeding, provided that Grantor gives Lsnder written notice of such claim and furnishes reserve~sor a surety bond for the claIm satisfactory to

Events AffecUng Guarantor. Any of the preceding events occurs with respect to any Guarantor of any of the Indebtedness or any Guarant~ dies
or becomes Incompetent; or revokes or disputes the validIty of, or lIabDity under, any Guaranty at the Indebtedness. Lender, at its option, may, but
shall not be required to, permit the'Guarantors estate to assume unconditionally the obligations arising under the guaranty in a manner satisfactory

demanding cure of such fallure: (a) cures the failure within fifteen (15) days: or (b) if the cure requires more than fifteen (15) days, immediately
inltlates steps sufficient to cure the failure and thereafter continues and completes all reasonable and necessary steps sufflclent.to prOduce
compUance as soon as reasonably practical. .
RIGHTS AND REMEDIES ON OEFAULT~ Upon the occurrence of any Event of Defaull and at any time thereafter, Lender may exercl~e anyone or

No ModlOcaUon. Grantor shall not enter fnto any agreement with tha holder of any, mortgage, deed of trust, or ather security agreement which
has priority over thIs Assignment by which that agreement Is modified, amended, extended, or renewed without Ule prIor written consent of

Investigation and Engineering Services Report for the proposed Child Center outpatient
facility in Springfield, Oregon. Our evaluation was completed in general accordance with
our proposal number 722-08-095 dated October 6, 2008 which was signed by you on
October 21, 2008.
Our investigation is intended to help you, the engineer, and the architect in designing
foundations and preparing plans and specifications for construction of the proposed
building.

GEOTECHNICAL INVESTIGATION REPORT
THE CHILD CENTER
SPRINGFIELD, OREGON
1 'INTRODUCTION
The new Child Center outpatient facility will be located at 3995 Marcola Road in
Springfield, Oregon. The site relative to surrbunding features is shown if) Figure 1.

Based on the information provided to us in an email dated September 28, 2008 and
. during a site visit on September 30, 2008, we understand the proposed development
will consist of the construction of one building. The building will be approximately 5,000
square feet and be constructed with wood frame on a concrete slab-on-grade. Detailed
structural loading was not provided at this time, therefore we.assumed maximum
column and wall loads will beon the order of 50 kips and 4 kips per linear foot,
respectively. The preliminary. site plan is shQwn in Figure 2, entitled "Schematic Site
Plan."
The conclusions and recommendations contained in this report are based on our
understanding of the currently proposed use of the project site, as derived from layout
drawings, written information, and verbal information supplied to us. Consequently, if
any changes are made in the currently proposed project, we may need to modify our.
conclusions and recommendations contained herein to reflect those changes.
The following paragraphs sUmmarize our site reconnaissance, subsurface investigation,
and geotechnical findings, conclusions, and recommendations for development.
2 PURPOSE.AND SCOPE OF WORK
Our geotechnical investigation provides recommendations for project design and
construction. The sRecific scope of our services is summarized below:

3 SITE CONDITIONS
3.1 Site Geology
The subject site lies within the southernmost portion of the Puget Lowlands that extends
from Vancouver, British Columbia to just south of Eugene. . The Willamette Valley
portion ofthe province is a narrow alluvial plain approximately 130 miles long and 20 to
40 miles wide (Orr and Orr, 1996).
Bedrock in the valley vicinity isthe Eugene Formation that dates to the late Eocene
period, about 35 million years ago. The Eugene Formation is a generally well-
consolidated to cemented, tuffaceous sedimentary rock deposited in a near-shore
marine environment.
Low energy streams and lakes that formed in the later Pliocene Epoch covered the
Eugene Formation with fluvial and lacustrine deposits of sill and clay to variable depths.
The rock was later folded and compressed during the Miocene and Pliocene Epochs
thatuplifted the Cascade and Coast Range Mountain Range~ and depressed.the
Willamette Valley (Yeats and others, 1991).
Uplift of the Cascade Mountains in the Pliocene Epoch steepened stream gradients
causing rapi.d erosion of the Cascade Mountains and deposition of thick gravel layers
that cover the Eugene Formation.
During the last ice age, 10 to 40 thousand years ago the Missoula Flood~ filled the
Willamette Valley to a depth of approximately 350 feet with silt, clay, sand and gravel
deposits (Allen and others, 1986).

3.2 Seismic Setting and Local Faulting' .
The Springfield area is subject to seismic events stemming from three possible sources:
ihe Cascadia Subduction Zone (CSZ) at the interface between the Juan de Fuca plate
arid the North American plate, intraslab faults within the Juan de Fuca plate, and crustal
faults in the North American plate. Maximum magnitude for a CSZ event is expected to
be in the range of Moment Magnitude (MW) 8..5 to 9.0: Known and suspected crustal
faults in the region have been characterized for the United States Geological Society
(USGS) and the Oregon Department of Geology.
There are no Quaternary crustal faults mapped in the immediate vicinity of Springfield.
The nearest mapped faults with quaternary displacement are approximately 20 to 40
miles away and include the unnamed faults near Sutherlin, the Upper Willamette River
fault zone, the Owl Creek fault zone, and the Corvallis fault zone.
The contribution of potential earthquake-induced ground motion from all known sources
is includedin the probabilistic response spectrum provided in the 2006 International
Building Code. Seismic site characterization and design recommendations in
accordance with the 2007 Oregon Structural Specialty Code are provided in the Seismic
Considerations section of this report.
3.3 Surface Conditions
The site is located at 3995 Marcola Road in Springfield, Oregon. The site is bordered to
the north and west. by Marcola Road and to the south and east by the McKenzie River.
The site is relatively flat with an approximate relief of one to two feet.
The site is vegetated with grass and sparse low'weeds.
3.4 Subsurface Conditions
We explored subsurface conditions at the site by completing two soil borings to depths
up to nine feet below the ground surface (bgs). The locations of the soil borings are
shown in Figure 2 entitled, "Schematic Site Plan". Soil samples were obtained for
classificatioll, verification, and laboratory testing. Descriptions of the field explorations,
laboratory testing program, test results, and the test pit logs are provided in Appendix A.

Soils encountered on site typically consist of surficial SILT (ML) with organic matter to
fourinches bgs. . Underlying the rooted zone is a plastic SILT (MH) to 7 Y, feet bgs.
From four inches to five feet bgs the material is reddish brown with some yellow to
yellowish brown mottling..At approximately five feet in depth the SILT became brown
with trace safid. The SILT with trace sand is underlain by well-gradedG~VEL (GW)
locally referred to as"bar run," which extend from approximately 7 Y, feet bgs to the
maximum depth explored of nine feet bgs. .
Our detailed interpretation of the soils encountered at the site is shown in the
exploration logs provided in Appendix A.
3.5 Groundwater
We did hot encounter groundwater in any of Qur soil borings on Novemb~r 3, 2008;
however, it is possible that seasonalliariations will cause groundwater fluctuations.
Additionally, ~erched water may be encountered in discontinuous zones \!I'ithin the near
surface soil. We recommend that the earthwork contractor verify actual groundwater
levels at the time of construction. Continuous groundwater or perched groundwater
may be near the ground surface later in the wet season or after. periods of prolonged
rainfall.
4 CONCLUSIONS AND RECOMMENDATIONS
4.1 General
Based on the results of o.ur reconnaissance, explorations; laboratory testing, and.
analysis, it is our opinion .that the site is suitable for the intended development. We find
no geotechnical conditions that restrict development other than the plastic SILT. We do
not recommend'supporting foundation loads directly on this material.
4.2 Site Preparation
We recommend intercepting anp diverting sources of surface or near-surface water
away from construction zones before stripping begins. Becau'se the selection of an
appropriate drainage system will depend on the water quantity, season, weather
conditions, construction sequence, and contractor's methods, final decisionstf&~~gceived:
drainage'systems are best made in the field at the time of construction.

The existing vegetation and the heavily rooted soil zone should be stripped and
removed from the site in all proposed building and fill areas and for a five foot margin
around such areas. Based on our explorations, the depth of stripping in these areas will
average about four inches. Greater stripping depths may be required to remove
isolated deeper zones. The actual stripping depth should be based on field
observations at the time of construction. Stripped material should be transported offsite
for disposal or used in landscaped areas. Grass and weeds should be rem.oved from all
building and paved areas. Depending on the methods used, considerable disturbance
and loosening of the subgrade could .occur during site grubbing. We recommend that.
soil disturbed during grubbing operations be removed to expose firm, undisturbed
subgrade. The resulting excavations should be backfilled with structural fill as .
described below,
A member of our geotechnical staff should observe the exposed subgrade and
excavations after stripping, grubbing, and site cutting have been completed to
, .
determine if there are areas of unsuitable soil. Our representative should also observe
a proofroll of areas .of the site that will receive structural fill. A fully loaded dump truck,
with a minimum gross vehicle weight (G'v.w.) of 40,000 Ibs, should be used to proofroll
and identify areas of excessive yielding. If areas of soft soil or excessive yielding are
identified, the soft soil should be excavated and replaced with compacted materials
recommended for structural fill. Proofrolling of the subgrade should not be performed
during wet weather or if wet ground conditions. exist. In these instances, the subgrade
should be evaluated using a soil probe. Soils that have been disturbed during site
preparation activities, Dr soft or loose zones identified during probing, should be
removed and replaced with structural fill.
4.3 Construction Recommendations
The fine-grained soils at the site can be. disturbed if construction occurs during the wet
season. If not carefully executed, site preparation, mass grading excavati.on, and utility
trench work can create extensive soft areas and significant repair costs can result. If
construction is planned for the wet season, the construction methods and schedule
should be carefully consider~d with respect to protecting the subgrade to reduce over
excavation of disturbed or softened soil.

subgrade below the granular haul road material.
4.4 Excavation Safety
Temporary earth slopes may be cut near-vertical to heights of four feet. Excavations
deeper than four feet should be performed in accordance with Department of Labor
Occupational Safety and Health Administration (OSHA) guidelines. Job site safety is
the responsibility of the 'project contractor.
In Federal Register, Volume 54, No. 209 (October 1989), the United States Department
of Labor, Occupational Safety and Health Administration (OSHA) amencjed its .
"Construction Standards for Excavations, 29 CFR, Part 1926, Subpart P". This
document was issued to better insure the safety of personnel entering trenches or
excavations. It is mandated by this federal regulation that excavations, whether they be
utility trenches, or footing excavations, be constructed in accordance with the new
OSHA guidelines. It is our understanding that these regulations are being strictly
. .
enforced and, jf they are not closely followed, the owner and the contractor could be
liable for substantial penalties.
The contractor is solely responsible for designing and constructing stable, temporary
excavations and should shor~, slope, or bench the sides of the excavations as required
. to maintain stability of both the excavation sides and bottom, The contractor's
. '
"responsible person", as defined In 29 CFR Part 1926, should evaluate the soil exposed
in the excavations as part of the contractor's safety procedures. In no case should
slope height, slope inclination, or excavation depth, including utility trencn excavation
depth, exceed those specified in local, state, and federal state regulations.
We are providing this information solely as a service to our client. PSI does not assume
responsibility for construction site safety or the 'contractor's or other parties' compliance
with local, state, and federCjI safety or other regulations.
4.5 Trench Backfill

Trench backfill should consist of a well-graded granular material with a maximum

200 Sieve. The material should be free of roots, organic matter, and other unsuitable
materials.
Backfill of the pipe embedment zone, including the bedding zone and pipe zone, should
be placed and compacted in maximum ,lifts of 6 inches. Trench backfill above the pipe
zone should be placed and compacted with a minimum of two lifts. A minimum cover of
3 feet over the top of the pipe should be placed before compacting with a hydrclUlic plate
compactor (hoe-pack): Within pavement or structural areas, trench backfill placed
within 3 feet of finished grade should be compacted to at least,95 percent of the
maximum dry density, as determined by ASTM D 1557 (modified proctor). In all other
areas, trench backfill should be compacted to at least 90 percent of the proctor,or as
recommended by the pipe manufacturer.
4.6 Structural Fill
The native soils are suitable for use as structural filf during prolonged periods of dry
weather. Du'ring wet weather construction, we recommend usinga granular material for
structural fills as described below.
Imported Fill Material: Typical structural fill materials include clean sand, gravel,
washed rock, crushed rock, quarry spalls, well-graded mixtures of sand and gravel
(commonly called "gravel borrow" or "pit-run"), and miscellaneous mixtures of silt, san'd,
and gravel. Recycled asphalt, concrete, and glass, which are derived from pulverizing
the parent materials, are also potentially useful as structural fill in certain applications.
The maximum particle size Jor structural fill should be restricted to 6 inches., . Fill
material placed during wet weather must generally consist of granular material with
fines content of less than 8 percent by weight.. Soil containing more than about 8
. percent fines (by weight) cannot be consistently compacted to a firm, unyielding
condition when the moisture content is more than about 2 percentage points above
optimum. For fill placement during wet weather, we recommend using granular material
with fines content of 8 percent or less based on the soil fraction passing the U.S. No,
200 Sieve.
Granular fill should be p1ace9 in lifts with a maximum uncompacted thickness of 12
inches. Fill material should be compacted to not less thim 95 percent of the maximum
dry unit weight density as determined by ASTM D1557. ' Date Received:

Regardless of material or location, all structural fill should be piaced over firm,
" ,
unyielding subgrade prepared in accordance with the "Site Preparation" section of this
report. A PSI representative should verify the condition of all subgrade before filling or
construction begins. Fill soil compaction should be verified by in-place density tests
performed during fill placement so that adequacy of soil compaction may be evaluated
as earthwork progresses, When fill material is inconsistent or when particle size is
greater than 1;1, inch in diameter, fill placement should be observed and compaction
evaluated by observation of proofrolls or the operation of heavy rubber tired
construction equipment.
4.7 Foundation Recommendations
Foundations bearing on 12 inches of structural fill overlying native.plastic SILT material
may be sized for a maximum net allowable soil bearing pressure of up to 2,500 pounds
per square foot (psf)., The allowable bearing capacity can be increased one-third for short-
term wind and/or seismic loading.
Exterior and interior continuous footings should have a minimum width of 24 inches and
be located at a depth of at least 18 inches below the lowest adjacent exterior grade to
provide frost protection. Continuous footings should be placed over 12 inches of'
compacted granular fill. The granular fill should be a minimum of 48 inches in width at
the base of the footing.
Spread pad footings should have minimum dimensions of 5 feet square and should be
placed over 12 inches of compacted granular fill. Gravel width at the bo!tom of pad
footirigs should be a minimum of seven (7) feet.
Consolidation of the overburden resulting from the foundation loads will result in some
:foundation settlement. Based on the soils we encountered, laboratory testing, and the
anticipated foundation loads, we estimate that foundation settlements should not
exceed one inch. Differential settlement between two adjacent columns should not
exceed 50 percent of the total settlement.
Allowable lateral friction resistance between the base of footings and the compacted
granular fill can be expressed as the applied vertical load multiplied by a coefficient of '_ ,~,

The foundation excavations should be observed by a representative of PSI prior to steel
or concrete placement to assess that the foundatio~ materials are capable of supporting
the design loads and are consistent with the materials discussed in this report. Soft or
loose soil zones encountered at the bottom of the footing excavations should be
removed to the level of firm soils free of deleterious materials or adequately compacted
structural fill as directed by the geotechnical engineer. Cavities formed as a result of
excavation of soft or loose soil zones should be backfilled with lean concrete or dense,
well-graded, compacted crushed rock.
4.8 Fioor Slab Support
We recommend the concrete floor slab-on-grade be supported on at least eight inches
of properly compacted, crushed, granular fill placed ~s described in section 4.6 of this
report. If the soil conditions allow, proofrolling as described in the "Site Preparation"
section of this report should be accomplished in order to identify any soft or unstable
subgrade soils that may need to be removed from the floor slab area prior to structural
fill placement. Additionally, the structural fill should be compacted in accordance with
the section 4.6'of this report, and also proofrolled to a firm unyielding condition.
If construction takes place during wet weather the rock section may need to be
increased from the eight inches recommended above. A representative of the
geotechnical engineer should assess the subgrade at the time of construction to make
this determination.
The crushed rock fill should provide a capillary break to limit migration of moisture
through the slab. If additional protection against moisture vapor is desired, a moisture
vapor retarding membrane may also be incorporated into'the design. Factors such as
cost, special 'considerations for construction and the floor coverings suggest that
decisions on the use of vapor retarding membranes be made by the architect and the

owner.
4.9 Seismic Considerations and Design Factors
For this site, we find no credible threat posed by soil liquefaction; surface fault offset; or
seismically induced landslides. Neither our investigations nor published reports by the
Oregon Department of Geology and Mineral industries (DOGAMll suggest an elevated

The contribution of potential earthquake induced ground motion from all known sources,
inCluding the faults described above, are included in the probabilistic ground motion
maps developed by the USGS and are implemented in the 2007 State of Oregon
. ,
Structural Specialty Code (OSSC)
In accordance with paragraph 1613.5 of the 2007 OSSC, mapped seismic acceleration
par~meters were obtained from'the United States Geological Survey for the project site.
The acceleration parameter Ss is equal to 0.63g and the acceleration parameter S1
equals 0.30g. The seismic soil profile for the upper 100 feet beneath the site conforms
to the characteristics of Site Class "D." The PGA (peak ground acceleration) for this site
is 0.262g. The return interval for these ground motions.is 2 percent probability of
exceedance in 50 years. .
5 RECOMMENDED ADDITIONAL SERVICES
It is recommended that PSI be retained to examine and identify soii exposures created
during project excavations in order to verify that soil conditions are as anticipated. We
further recommend that structural fills, if any, be continuously observed and tested by
our representative in order to evaluate the thoroughness and uniformity of their
compaction. If possible, samples of fill materials should be submitted to our laboratory
for evaluation prior to placement of fills on site. Costs for the .recommended
observations during construct~on are ,beyond the scope of .this current consultation.
Such future services would be at an additional charge.

FIELD EXPLORATION PROGRAM
General
We explored subsurface conditions at the site by completing two soil borings (B-1 and
B-2) at the locations shown on the attached Site Schematic Map. Soil borings were
'performed yvith a CMA 75 rubber-tired drill rig, The soil borings were'advanced to 8 Y:.
to 9-feet below existing site elevations.
The soil borings were drilled on November 3, 2008. The drill rig was operated by Ken
Kreitz with PSI.
A geology staff member of PSI was present throughout the explorations to record soil,
rock, and groundwater conditions encountered and to obtain select soil.sample~ for.
laboratory testing.
Sampling Procedures
Disturbed samples were collected from the split-spoon sampler, Samples were
identified, placed in sealed plastic containers, and transported to our laboratory for
further classification and testing,
Field Classification
,
Soil samples were classified in the field in general accordance with the American
Society for Testing and Materials (ASTM) 02488 Visual-Manual method. Where used,
the term "general accordance" means that certain local and common descriptive.
practices and methodologies have been followed. Consistency, color, relative moisture,
degree of plasticity, peculiar odors and other distinguishing characteristics of the
samples were noted. The terminologies used in the soil classifications are defined in
the Soil Classification,

Summary exploration logs are presented in this'appendix. The left-hand portion of the

logs gives our interpretation of the soil encountered in the soil borings, sample locations
and depths, and groundwater information, The right-hand, graphic portion of the boring
. logs shows the uses soil classification, Atterberg limits values, and sample water
contents.
Soil descriptions and iriterfaces between soil types shown in the logs are interpretive,
and actual changes may be gradual.
LABORATORY TESTING
General
The soil and rock samples obtained during the field explorations were examined in our
, .
laboratory. The physical characteristics of the samples were noted and the field
classifications were modified, where necessary. During the course of the examination,
representative samples'were selected for further testing. The testing program included
visual-manual classification, sieve analysis, natural water content, and Atterberg limits.
The testing procedures and results of the tests are summarized in the following
paragraphs. Unless noted otherwise, all test procedures were in general accordance to
applicable ASTM standards. "General accordance" means that certain local and common
descriptive practices and methodoiogies have been followed.
Natural Water Content
Natural moisture content determinations were made on selected samples. The natural
moisture content is defined as the ratio of the weight of water to dry weight of soil,
expressed as a percentage. The results of the moisture content determinations are
presented on the test pit logs in this appendix.
Atterberg Limits
Atterberg limits were determir)ed on selected samples in accordance with ASTM D-
4318. This analysis aids in soil classification. The results of the Atterberg limits test,
. which include liquid and plastic limits, are plotted in the Atterberg Limits' Results page,
located in this appendix.

Sieve Analysis
Two sieve analyses were performed in accordance with ASTM C-117. This test aids in
classification of soils and provides data pertinent to the soils' engineering properties.
Soil Classification
. Soil samples obtained in the field were examined in the laboratory to confirm field
. classifications. Where warranted, we changed the field classification to more closely'
agree with guidelines presented in ASTM D-2488, Standard Practice for Description and
Identification of Soils (Visual-Manual Procedure), with PSI's descriptive terms such as
the relative density or consistency of the soil deposits, and with current local
engineering practice. In determining the soil type (that is, gravel, sand, silt or clay), the
term that best described the major portion of the sample was used.

Storm Drainage Evaluation
The Child Center
Introduction
This evaluation was prepared to summarize the drainage system proposed for the Child Center
development. This'storm drainage evaluation will deteimine the type and size of water quality
treatment devices that are acceptable to the City of Springfield. The Child Center is located on the
south side of Marc 01 a Road and west of the McKenzie River at the intersection of Marcola Road and
Hayden Bridge Road in Springfield, Oregon. The proposed project site is comprised of two tax lots,
TM170220 TL 700 and TM170230 TL 100.
Exis:ting Conditions.
The existing site is bounded by Marcola Road to the north and west and the McKenzie River to the
south and east. The site consists of a existing buildings, driveway and landscaping. The impervious
surfaces consist of roofs and concrete sidewalks and asphalt driveway. The pervious surfaces consist
of landscaping. Runoff from the. existing site sheet flows to the west towards Marcola Road and also
to the east to the McKenzie River. There is apparently no public storm system'serving this property.
According to the Soils Survev of Lane County. Orel!on. by the Natural Reso1J!ce Conservation Service,
the soil at the site is Fluvents (Soil Number: 48, Hydrologic GrouR: C), Malabon silty clay loam (Soil
Number: 75, Hydrologic Group: C) and Nekia silty clay loam (Soil Number: 89C, Hydrologic Group:
B). Refer to Appendix A for existing conditions plan. Refer to Appendix B for soils map information.
Proposed Site Conditions
The proposed project includes the addition of a single story 5,177 sf outpatient/office building with
associated parking, sidewalks and landscaping. The parking and drive aisles will consist of asphalt
pavement and the sidewalks will be concrete.
Water Quality Treatment and Storm Water Conveyance System
Eugene Water and Electric Board (EWEB) has expressed concern for the quality of the stormwater
runoff from the proposed site. The EWEB water intake station is just to the east of the property on the
McKenzie River. Reasonable efforts to protect any water entering the McKenzie River from the site
will be put forth to accomplish this goal. Infiltration testing was conducted and the site was
determined to have adequate infiltration rates to serve the proposed improvements. See Appendix B
for the infiltration testing report. A vegetated infiltration swale will be used for disposal of the 25 year
storm event. The vegetated infiltration swale will be landscaped with appropriate vegetation and
topsoil to allow treatment and infiltration of stormwater. There are two vegetated infiltration basins,
one on either side of the proposed east driveway approach. The two vegetated infiltration basins have
varying infiltration rates. See Appendix C for the proposed Drainage Basin Map for location of
infiltration test pits. The east swale had lower infiltration rates than did the west swale. The location
of the swales determined the infiltration rates that are being used. See Appendix D for the Infiltration
Swale Routing Data. The two vegetated infiltration swales are hydraulically connected by a 12" storm
pipe and are acting as a single swale.
The parking lot will sheet flow runoff to the vegetated infiltration swale. However, there are some
portions of the parking area that will require being directed to double chambered catch basins with
. filter inserts and piped to the vegetated infiltration swale. The double chambered catch basins with
filter insertswil),help.iIflprove the water quality of the storm \yater before it enters the vegetated
infiltration swale. The building will drain to the infiltration swale via storm pipe. An emergency
overflow outlet is provided for storm events larger than the 25 year storm. The existing buildings drain

Appendix C for the proposed 'Drainage Basin Map.
The vegetated infiltration swales have 3:] side slopes, The swale bottom is propbsed at elevation
477.00 and a top elevation'of 480.00. The runoff flowing to the vegetated infIltn'ttion swales during
the 25 year storm event is ] .70 cfs using the Santa Barbara Urban Hydrograph method. A factor of
safety of2 for the tested infiltration rateis being used in the design of the irrliltration facility. The' 25
year storm is routed through the vegetated infiltration swale and the depth of the stormwater in the
vegetated infiltration basin is 2.28 feet and reaches an elevation of 479.28. See Appendix D for
stormwater runoff, routing and infiltration swale stage-storage-outflow calculations. See Appendix E
for InfIltration swale section and double chamber catch basin with filter insert.

Description
Hydrologic soil groups are based on estimates of runoff potential. Soils are
assigned'to one of four groups according to the rate of water infiltration when the
soils are not protected by vegetation, are thoroughly wet, and receive precipitation
from long-duration storms.
The soils in the United States are assigned to four groups (A, B, C, and D) and
three dual classes (AID, BID, and CID). The groups are defined as follows:
GroupA Soils having a high infillration rate (low runoff potential) when thoroughly
wet. These consist mainly of deep, well drained to excessively drained sands or
gravelly sands. These soils have a high rate of water transmission.
Group B. Soils having a moderate infiltration rate when Ihoroughly wet. These
consist chiefly of moderately deep or deep, moderalely well drained or well drained
soils that have moderately fine texture to moderately coarse texture. These soils
have a moderate rate of water transmission.
Group C. Soils having a slow infiltration rate when thoroughly wet. These consist
chiefly of soils having a layer that impedes the downward movement of water or
soils of moderately fine texture or fine texture. These soils have a slow rate ofwaler
transmission.
Group D. Soils having a very slow infiltration rate (high runoff potential) when
thoroughly wet. These consist chiefly of clays that have a high shrink-swell
potential, soils that have a high water table, soils Ihat have a c1aypan or clay layer
at or near the surface, and soils that are shallow over nearly impervious material.
These soils have a very slow rate of water transmission.
If a soil is assigned to a dual hydrologic group (AID, BID, or C/D), the first lelter is
for drained areas and the second is for undrained areas. Only the soils that in their
natural condition are in group 0 are assigned to dual classes.
