Performed continuous inspection during the installation of twelve (12) 1/2" x 5 1/2"
vertical Simpson Wedge-All anchors (ICC - ES ESR 1396) into slab on grade for PT
sill plate attachment. Verified minimum embedment of 3". Note: anchors were
unable to be tightened to manufacturer's required installation torque of 60 ftllbs.
due to nut and washer compressing into wood as nut is tightened. Torque
reached was approximately 25 ftllbs. as checked with FEl's calibrated torque
wrench.
Toby DeMasters (Mcintyre Construction) was asked to notify our office when additional
services are needed.
To the best of our knowledge, the work inspected was in accordance with plans dated
08-19-08 and applicable' workmanship provisions of the I.B:C., except if noted above,
