Performed weld inspection on #8 rebar to embed plates (4 total) as noted on sheet S2, Fit~up of weld joints were
inspected for workmanship, In Progress welding was monitored to the fabrications FCAW welding procedure
specifications, Weldments were inspected for size, length, profile and locations, To the best of our knowledge, the work
inspected is in conformance with design drawings dated 9/25/09, specifications letter from engineer and applicable D1.4
codes, Contractor was asked to notify our office when additional services are needed,
