Dear Mr. Evans:
Your request for an extension of the permits on behalf of the owners of the
property for the sanitary and storm sewer permits for the extension lines located
in phase 1 and phase 2 for Mountain Gate Subdivision, City Job Number
COM2004-01120 has been reviewed and approved. This extension may only
be granted one time and will expire on February 22, 2006.

RE: Permit No. Com 2004-01120
Dear Ms. Hopper,
This letter is a request for extension of the plumbing permit referenced above. The
permit was issued for private storm and sanitary sewer lines within the MountainGate
Phase I and 2 development.
The permitted sewer lines have been installed except those associated with two joint use
driveways serving multiple lots. Construction of the driveways has been delayed until
the one serving Lots 40, 4\ and 42 is built with the construction of Phase 3.
The driveways, including the sewer extensions will be completed by October 15, 2005.
