Due to a pending land use partition approval, the street address of your existing structure
has been changed in order to allow for the street address numbering of the new

structure(s). Your property currently and commonly known as 1473 5 Street,
Springfield, Oregon, also known as Lane County R.L".,,~ce Number 17032640, Tax Lot
Number 001300 has been changed to 1483 5th Street. The additional lot created by the
partition approval will be known as 1487 5th Street, also referred to as Reference Number
17032642, Tax Lot 01200.

This change will become effective 30 days from the date of this letter (November 13,
2009), or, by the required address change date by Springfield Planning which will be
prior to the 30 days effective date. Please change your street address numbers on your
home and on your mailbox (if applicable) by this date.
