Dear Mr. Myles:
On August 9, 2004, our office issued pennits for a re-roof on an existing structure located
at 1147 Main Street, Springfield, Oregon. While calculating the fees for that pennit, the
plan reviewer neglected to include the 7% State Surcharge of$13.52 and the 10% City
Administrative Fee of$19.32, leaving a balance due of$32.84 for this project. I am
enclosing a copy of the pennit that was issued and a copy of the original receipt for your
reference.
Please pay the amount due prior to requesting your final inspection for this project. I
have enclosed a prestamped envelope for your convenience if you wish to make payment
by mail, or you are welcome to make payment in person at our office. Our office hours
are 8:00 a.m. - noon and from 1 :00 p.m. - 3:00 p.m. Monday through Friday. I sincerely
apologize for any inconvenience this may cause you.

Description
UltraGard SRT-45, SRT-60, SRT-Extra 72 and SRT,Extra 80 are
polyester reinforced thermoplastic polyolefin membranes ,
designed for use in mechanically fastened and adhered roof-
ing membranes, These JM TPO membranes come in 8, 10 and
12 foot widths,
Use
UltraGard SRT-45, SRT-60. SRT-Extra 72 and SRT-Extra 80 mem-
branes can be installed in new, reroof (tear-off) and recover roof
constructions. In recover construction, if the existing roof is
sound, the Johns Manville (JM) roof system can eliminate the cost
of disposing of the original-roof, UltraGard SRT-45, SRT-60, SRT-
Extra 72 and SRT-Extra 80 membranes are installed over approved
cushioning layers of insulations, when required, and secured to
the roof deck utilizing either mechanical fastening methods that
have been tested for wind uplift in both dynamic and static pres.
sure vessels or adhering the membrane to an acceptable sub-
strate. JM TPO membranes are thermoplastic; therefore, when
rolled out onto the roof substrate, they can be easily welded into
one homogeneous sheet using hot air welding procedures.
Limitations
UltraGard SRT-45, SRT-60, SRT,Extra 72 and SRT-Extra 80 mem,
branes are mechanically fastened or adhered to the structural roof
deck according to Factory Mutual (FM) requirements, publiShed
standard fastening patterns or adhesives recommendations.
Composition and Materials
UltraGard SRT-45, SRT-60, SRT,Extra 72 and SRT-Extra 80
membranes are composed of a top and bottom film with a poly-
ester reinforcement for added dimensional stability and supe-
rior tear strength. The top and bottom films are formulated with
an ultra-violet resistant thermoplastic polyolefin.

Applicable Standards

JM Mechanically Fastened and Adhered TPO Roof Systems are
classified by UL (Underwriter's Laboratories, Inc.) and Factory Mutual.

Surface Preparation
The entire area to be roofed must be inspected by a JM applicator
to determine if installation of a JM roof system is acceptable, The
substrate must be clean, smooth, dry, free of flaws. debris and
other irregularities. I
Placement of JM Membrane
JM specifications require treated wood naile.rs to be installed
according to Factory Mutual current recommendations'in Loss
Prevention Oata Sheet 1-49 and currentJM Detail Drawings,
Insulation must neatly f~ around all roof penetrations and projections,
Insulations used must be accepted by JM for use in the system
to be installed. Fastening of the insulation must be in strict
accordance with JM standard fastening requirements andlor
any applicable code requirements, whichever is most ~estrictive
or required by the building owner,
Method of Attachment
The membrane may be attached using either the mechanically
fastened or adhered method. For mechanically fastened systems,
JM offers a disk and fastener system which is installed.within the
laps at increments as determined by the applicable building
codes, specification criteria or printed guide specifications
and/or technical data, For adhered systems, the membrane is
adhered to the appropriate substrate with JM Membrane Adhesive.
