Amy: Just a heads up that I am sending you a DWP application packet via internal mail for the Willamette Valley Cancer
Center. The WVCC is relocating to the Riverbend Pavilion at Sacred Heart Medical Center. The application packet
includes a CD with all the HMIS and MSDS information in .pdf format. Please review the DWP packet and let me know if
they have provided all the necessary information for their application. Thanks
