Subject: Occupancy Inspection at 3834 MBin Street, Springfield, Oregon.
Proposed Use: Retail & Wholesale Bingo Supplies

At your request, the Springfield Building Safety Division recently
conducted an inspection of the building(s) at the above address. The
purpose of the inspection was to detennine the suitability of the
building(s) for the proposed use as indicated.
Based on the proposed occupancy, the existing conditions which are
mentioned below do not meet the minilllum Building Safety Code requL ~,~.ts.
Cv..~v~ive measures must he taken prior to occupancy to install, repair,
replace or modify the following items in oreer for the building to conform
to applicable safety codes:
Structural
1. The ~~r~~ed insulation facing in the upstairs room shall be protected or
removed.
2. Provide two 5 lb. type ABC fire extinguishers in locations ~u- _,~:. by the
City Fire Marshal.
3. Guardrails are required on all balconies, landings, floors or roofs which
are used for purposes other than servicing the building and which are IIIOre
than 30" above grade or the floor below. Tbe guardrail shall be at least
42" high and shall have intermediate rails spaced at intervals that will
not allow a 6" sphere to pass through (up to 12" rail spacing _y be used
in areas not accessible to the public). Open stairs shall be similarly
protected except that the height of the handrail shall be between 30" and
34" above the stair nosings. The ends of the handrails shall return to
the newell post or wall.

Electrical
"
4. Improper splicing of electrical w1r1ng was observed. The splices or
terminal end of electrical wiring must be contained in an approved
junction box, with a cover, to prevent possible electric shock or fire
from'! ~ur .~ed electrical wiring or short-circuit conditions.
5. All preakers in electrical panels shall be r,vr~.ly labeled.
6. Where combustible materials have been added to the walls, electrical boxes
shall be brought to within 1/8" of the surface or extension rings shall be
used.
7. Prop,erly secure the 220 volt . ~_~r~acle in the storage room.

8. Broken or missing cover plates were noted on electrical outlets, switches '
andi6r junction boxes which require repla_~~t to reduce the possibility
of electrical shock or fire.

9. Electrical light fixtures were found that were in~..~. ..hIe, damaged or
miss'ing and shall be either repaired or replaced. '
Plumbing
10. Toi~ets in public rest rooms shall be provided with open front toilet seats.
11. Disconnect the water supply lines to the counter top lavatory sink in the
dis~lay area or install a new drain line for this fixture.
Building permits must be obtained for the above items which involve
repairs or modifications to the structural, electrical, plumbing or
mechanical systems of the building and for any additions or revisions yo*
wish to make to the building. '
The abo~e items are requi. ~~ts for the existing structure only. other
items such as parking, paving, site improvements, sidewalks, etc., have
not been addressed as part of this inspection, and may be required.
