RE: Revocation of permit for parking lot paving at 3520-22 Main Street, Job #870121
Mr. Bourne:

The intent of this letter is to give written notice that the above referenced paving
permit was issued in error, and is hereby revoked under the authority of Section 303(e)
of the Sprin9field Building Code Administrative Code, which reads as follows:
(e) SUSPENSION OR REVOCATION. The Building Official may, in writing, suspend
or revoke a permit issued under the provisions of this Code and the Specialty
Codes whenever the permit is issued in error or on the basis of incorrect infor-
mation supplied, or in violation of this Code, the Specialty Codes or any
statute, regulation or ordinance of any governmental subdivision of the State.
Prior to issuance of this permit, two approvals"are required',throu~h'the'f6llo0ing
review processes:

1- Since a change of occupancy type is intended, the new use shall be reviewed
and an approved plan developed through the Site Review process. Application forms
and attached explanation of this review process have been supplied to the prospec-
tive tenant. Contact Cindie Harmon at 726-3759 regarding the Site Review process.
2- A paving and drainage plan shall be submitted along with an application for
appropriate permits for construction, to the Springfield Building Safety Division
for plan review and approval. An approved copy of the construction documents
will be retained at our office, and one returned to the applicant when the permits
are issued. The approved plans shall be retained at the job site during the
construction project, and may not be removed until the required inspections are
completed and final approval of the project is granted by the City Building
Inspector. Contact Lorne Pleger at 726-3669 regarding the Plan Review process.

and correct, and I further certify that any and all work performed shall be done in accordance with the Ordinances of the City
of Springfield and the laws of the State of Oregon pertaining to the work described herein, and that NO OCCUPAUCY will be made
of any structure without the permission of the Building Division. I further certify that my registration with the Builder's
Board is in full force and effect as required by DRS 701.055, that if exempt the basis for exemption is noted hereon, and that
only subcontractors and employees who are in compliance with ORS 701.055 will be used on this project.

To avoid design or construction delays. Building
Division Staff must be able to contact appropriate
persons regarding deSign information or job site
corrections, etc.
II. Abbreviated Plumbing, Mechanical, & Electrical Schedules
A. Except where blank spaces occur in the description
portion of the Mechanical and Electrical Schedules,
the applicant need fill-in only the No. Boxes adjacent
to the appropriate item(s) to be installed
B. Full Plumbing, Mechanical, and Electrical Schedules
are available at the Building Division
1. To conserve space on the permit form the schedules
have been abbreviated
2. If the item(s) to be installed are not covered on
the abbreviated schedules you should consult the
full schedules
C. BUILOING DIVISION STAFF WILL FILL OUT ALL FEES AND
CHARGES ON THE SCHEDULES
O. As noted on the CAP, the label must be delivered to the
electrical contractor for signature by his electrical
supervisor. The general contractor is.not authorized
to sign the electrical label. ---
III. Applicant to sign and date
Whenever possible, the initial application will be used as
a worksheet only. Where possible, Building Division Staff
will prepare a type written copy and return it to the
applicant at the time the actual permit is issued for his
signa ture.
IV. Fees and Charges
Plan check fees are due and payable at the time of the
application, and no plans will be processed until these
fees are paid. All other fees and charges are due and
payable when the permit is issued.
