No.: P0272-334-0809 Type of Inspection: Soils Observation Weather: Clear FieldObservations and Comments As requested, a PSI representative was on-site to inspect the following: a proof roll at finished rock grade for all parking lot and driveway pavement areas, The proof roll was conducted instead of field density testing because the crushed rock had varying amounts of gravel, was from different sources, and therefore a representative proctor could not be obtained. The proof roll was conducted using a fully loaded tandem axle dump truck weighing approximately 55,000 pounds. No defiections (pumping) were observed under the wheel loads. Also no wheel tracking occurred, indicating a firm, stable, non-yielding, compacted rock grade with the exception of the following areas: an area measuring 8'x8', 20' by 25' and 20' by 25' (See Attached Site Plan), At those locations, moderate defiectioosoccurred and rutting was observed indicating an unstable, yielding surface. Recommended stabilizing these areas prior to paving, stabilizing might include re-compacting or over excavating and re-compacting, To the best of our knowledge, the work inspected above was io accordance with the building department approved design drawings, specifications and applicable workmanship provisions of the International Buiidinq Code.
