Attached is an electronic copy .of the REVISED FINAL HMMP for the Walgreens Clinical Pha~macy in Springfield, Oregon.
All comments made by SUB and the City are addressed in the REVISED FINAL HMMP.
The HMMP text changes/revisions are as follows:
. HMMP, Page 2: Section 2.1 has been added to provid'e Facility Staff Designated to 'implement the HMMP and
Its Requirements;
. HMMP, Pages 5.:&, Section 4.1: Detailed information concerning secondary contain(nent devices for
pharmaceutical items and medicines; . I!
. HMMP, Pages 6, Section 4.1: Detailed information concerning secondary containment devices for regulated
hazardous waste (in the event that it is generated);
. HMMP, Page 11, Section 7.0: Text has been modified to address that the Facilityis;unlikely to generate
regulated hazardous waste at this time. However, in the event that such waste is generated and stored at the
Facility, it will be inspected as stated in Plan Section 7,2; and "
. HMMP, Page 11: Section 7.2 has been added to address Hazardous Waste Storage, Monthly Inspections, in the
event that regulated hazardous waste is generated.
Three hard copies of the REVISED FINAL HMMP are in the process of being reproduced. They will be sent to the
attention of Andy Limbird at the City Planning Division. These hard copies will arrive NL T June 17, 2009.

After lengthy discussions between Walgreens representatives, Springfield Utility Board (SUB) ,Water Quality staff and

Springfield staff, the City's Building Official has issued occupancy for the Walgreens clinical pharmacy located at

RiverBend Pavilion, This allows the pharmacy to begfn operations and open to the public. All other conditions of
existing land use approvals (including the recently-issued Drinking Water Protection permit, Case DRC2009-00019)
remain unchanged and in effect.
The occupancy may be rescinded and is subject to a satisfactory final inspection by SUB Water Quality on or around
Tuesday June 16, 2009. Final arrangements for the site inspection are to be made between Walgreens and the SUB
Water Quality Protection Coordinator (Amy Chinitz, S41-744-374S). Additionally, a revised Hazardous Materials
Managem~nt Plan (HMMP) for the Walgreens clinical pharmacy is being prepared for submittal to SUB Water Quality
Protection and the City of Springfield. Planning staff requests that three (3) hard copies of the final HMMP are
submitted along with an electronic copy in .pdf format. The electronic copy can be sent to the project planner at:
