Jim,
Here are a couple additional comments from the City Surveyor's office regarding your 5/13/09 submittal of I. he plat,
declaration of restriction and Improvement Agreement exhibit:

. On the Declaration of Restriction, the 2" notary acknowledgement has the named person as "the above named
Gayle Trustee of....". It needs to state the full name, "Gayle D. Helfrich".
. The Improvement Agreement is fine, we have incorporated the new description into the already signed and
received document.
PLAT:
. In the lower left hand corner of I. he plat, the City Case number needs to be "2008-00039", not "00390"
. Also, on the "planning Director" signature line it should state "City of Springfield Planning Director".
Also, please submit a copy of the current update to the title report when you get it, and the'copy of the Boundary Line
Agreement if it is an exception on the title report.
Please note these are in addition to any comments the County Surveyor has, which you should be receiving shortly.
When all requested edits are satisfied, you may submit the final plat application packet to the City, as per the guidelines
and ~hecklist detailed on the application document.
