Dear Business Owner/Manager:
In response to a request from the Fire Department, an inspection of the electrical
wiring and equipment was conducted at the above address. The items listed on the
att~ched sheet were discovered during the inspection. Each of the items presents
a potential hazard to life and property and constitutes a violation of the Oregon
Electrical Specialty Code (O.E.S.C.). As such, we request that action be taken to
correct or eliminate these hazards within 30 days of this notice.
Since the building in question:is leased, Oregon law requires that corrections be
made by an electrical contracting firm licensed by the State to preform such work
(Oregon Administrative Rule 22-110). For your protection, you should make sure
that the electrical contracting firm chosen obtains the necessary permits and in- '\",'
spections from this office.

1) Conduit systems shall be continuous between boxes or other enclosures for
physical protection of the conductors and to assure grounding continuity.

2) All cord, fixture wire and non-metallic cable shall be replaced with conduit
as the wiring method. O.E.S.C. 400-8
3) Where cord connected equipment is necessary, the required outlet shall be
conduit supplied and located within the limits of the manufacturers instal-
led cord with grounding conductor. O.E.S.C. 400-7, 8 & 9
4) All electrical equipment, unless double insulated, shall be bonded (grounded).

area is to be kept clear from obstructions at all times. O.E.S.C. Table 110-16
