Dear Business Owner/Manager:
In response to a request from the Fire Department, an inspection of the electrical
wiring and equipment I~as conducted at the above address. The, items listed on the
attached sheet were discovered during the inspection. Each of the items presents
a potential hazard to life and property and constitutes a violation of the Oregon.
Electrical Specialty Code (O.E.S.C.). As such, we request that action be taken to
correct or eliminate these hazards within 30 days of this notice.
Since the' building in question' is leased, Oregon law requires that corrections be
made by an electrical contracting firm licensed by the State to preform such work
(Oregon Administrative Rule 22-110). For your protection, you should make sure
that the electrical contracting firm chosen obtains the necessary permits and in-, ,,,:1.
spections from this office.

Dear Business Owner/Manager:
In response to a Fire Department request, an inspection of the electrical w1r1ng and
equipment was conducted at the above~:~~~J address. Revealed were the following at-
tached list of defects that are in violation of the Oregon Electrical Specialty Code
(O.E.S.C.) and are considered hazardous to personel and property. Due to the poten-
tially hazardous nature of the altered and additional wiring, it will be required
that the corrections be made within 30 days of notice and all wiring shall be performed
by an electrical contractor as required in Oregon Administrative Rules 22-110.
Permits'may be obtained at the Springfield Department of Public Works, 225 North 5th
Street. Thank you for your cooperation in this matter.
