Field Observations and Comments
Performed continuous inspection during the installation of #4 epoxied rebar dowels in the following locations: (4) 5/8"
bolts at north shear wall, (4) #4 rebar at west footing and (2) #4 rebar at stem walls. Prior to installation the predrilled
holes were cleaned using a nylon brush and compressed air. Verified minimum. embedment depth of 10 inches for 5/8"
and 4 inches for #4. The above listed epoxy anchors were installed uslng..a Simpson Set-XP epoxy with ICC #XP22
expo 5/31/11. Contractor stated the nuts would be tightened after the epoxy has cured. The epoxy anchors were
installed in accordance with the manufactures directions and iatest revisions dated 7/21/09. Contractor was asked to
notify our office when additional services are needed. To the best of our knowledge, the work inspected above was in
accordance with the building department approved design drawings, specifications and applicable workmanship
~rovisions of the International Buildinq Codes.
