Field Observations and Comments
A PSI representative was on site at Thurston Elementary School Public Improvements to perform a proof roll on
driveways off Thurston Road. The proof roll was conducted using a loaded ten-wheeled dump truck approximately
50,000 pounds. There were no deflections or pumping observed under the wheel loads and no wheel tracking on rutting
indicating a firm and stable subgrade at this time. To the best of our knowledge, the work inspected above was in
accordance with the building department approved design drawings, specifications and applicable workmanship
provisions of the International Buildinq Codes.

A PSI representative was on site at Thurston . Elementary School to do site observation. PSI talked with Mark with
Hyland Construction on the Public Improvement. Mark said that there would be a sidewalk and two (2) curb cuts that
are coming up in the future. Mark said he would call PSI when services are needed. To the best of our knowledge, the
work inspected above was in accordance with the building department approved design drawings, specifications and
applicable workmanship provisions of the International Buildinq Codes.

Field Observations and Comments
Witnessed a proof roll of the driveway on the west side of the building (See Attached Drawing) at finish grade of base'
rock. The proof roll'was performed with a loaded dump truck weighing approximately 50,000 Ibs. Only slight tracking
and no pumping was observed under the wheel loads indicating a firm and stable grade. To the best of our knowledge,
the work inspected above was in accordance with the building department approved design drawings, specifications and
applicable workmanship provisions of the International Building Codes.

Field Observations and Comments
As requested, a representative of PSI was on site to verify pier excavation and embedment into "bar run". Eleven (x11)
piers were excavated to depths ranging from 12 feet to 14 Yz feet below ground surface. Upon our arrival the holes had
been drilled and the delta crew were preparing to drop rebar cages. Visually, the holes appeared to penetrate two (2) to
three (3) feet into the "bar run" material. Depth of excavation from south to north are as follows: 12 Yz (gw), 12' (gw), 12
'%' (gw), 12 Yz' (gw), 12', 12' (gw). 12', 12 Yz', 14 Yz', 12 Yz' (gw) and 13' (gw). (gw) indicates ground water was observed
in the base of excavations. To the best of our knowledge, the work inspected above was In accordance with the
buiiding department approved design drawings, specifications and applicable workmanship provisions of the
.International Buildinq Codes.
