Dear Mr. Hooten:

At the request of the Fire Marshal's office, the City of Springfield Building Division
made an inspection of the electrical wiring installation at 4090 Main Street, on February
20, 1981. It was found that all additional wiring and alterations made since the original.
installation have been made with no regard to"the Oregon Electrical Safety Standards.
(OESS).

Due to the potential hazards involved,it'isrequired that
corrected in the immediate future:
1. Install --oVercurrent protection in accordance with 'conductor.'ampacity in all panelS _
(240-2, OESS).
2. Bond service conduits. Approved. clamps will be acceptable ~ (250-32, OESS).
3. Properly install--covers on all panels. Covers to be tight to panel enclosures and
prpperly support the breakers - (370-52 (e)&(f), OESS).
4. Seal all open spaces in panels- (373-4,OESS).
5.. Clearly identify all circuits in panel dead fronts, and extended ,switch assemblies _
(110-22 OESS). .
6. Additional wiring shall ,be 'installed in compl iance with the 'existing conduit install a- .
hon. Remove all cord and fixture wiring ~. (Oregon Administrative Rule (OAR) 22-135).

7. Secure all junction and outlet boxes below food'preparing equipment -(300-11, OESS).
8. Install covers on all open junction boxes in all areas - main floor and both ends
of attic -(370-18 (c) OESS).
9. Replace defective duplex receptacles'''; (110-3, OESS).
10. Install cover plates on al.l receptacleands\1itchoutlet boxes -(370-15, OESS).
11. Remove open wiring to all refrigeration compressors, and secure and cover spl ice
boxes to motors - (110-8, OESS).
12. Properly mount and wire transformers .~. (1lD-8; 450-7, OESS).
13, .Remove timers now installed in dishwasher control center,and install in an
, approved uti'l ity box adjacent to control center - (373-11, OESS).
14. Install cover on dishwasher control center- (370~18 (c). OESS).
15. Properly install light in attic - (liO~34 (d), OESS).
16. Properly terminate cords from foodprepari'ng equipment into juncti on boxes with
. grommet type connectors - (400-10, OESS).'
17. All electrical equipment shall be bonded or grounded - (250-75, OESS).
18. Remove open wiring to outside cover light fixtures - (110-8, OESS).
19. Repair defective fixtures in all areas - (110-3, OESS).
20. Disconnect circuits to ground and. roof spot lights - (110-3' (8), OESS).
21. Properly install burial cable to.garage - '(300":5, OESS).
